Abstract

General Background Children born with congenital anomalies require specialized health interventions to optimize their communication development. Specific Background Patients undergoing palate reconstructive procedures often face functional articulation problems and require continuous therapy through a Home Exercise Program guided by their families. Knowledge Gap Existing literature provides limited exploration regarding the actual achievement rates and detailed experiences of family involvement in guiding these communication exercises at home. Aims This study investigates the implementation and outcomes of caregiver guidance in the articulation training process for children at the CLP Center Surabaya. Results Qualitative interviews with families, teenage patients, and therapists reveal that guided home practice significantly builds children's confidence, discipline, and focus during communication training. Furthermore, while the provision of training modules improved family understanding, primary obstacles included limited caregiver time, patience deficits, and difficulties in maintaining the child's practice interest. Novelty The research uniquely highlights how a structured companion model utilizing specific training modules shifts the primary therapeutic responsibility from clinical practitioners to empowered families. Implications These findings offer a comprehensive framework for other therapeutic institutions to integrate structured family bonding and home exercise protocols into standard clinical care. Highlights: Family guidance during home exercises significantly builds child confidence and discipline during articulation training. Major challenges include time limitations, caregiver impatience, and maintaining motivation for continuous practice. Utilizing structured modules shifts therapeutic responsibilities toward home caregivers, supporting communication recovery.
