Abstract

Safety and operational efficiency are fundamental pillars of the aviation industry, where the reliability of each aircraft component plays a vital role. One crucial component of the Airbus A330 fuel system is the Twin Motor Actuator (FRH010041), which controls the crossfeed valve between wings. Failure of this component may disrupt fuel distribution and significantly impact flight safety. This study aims to analyze the root causes of 40 repetitive failure cases recorded at PT GMF AeroAsia from 2020 to 2024. The urgency is reinforced by its classification under the Minimum Equipment List (MEL) category Repair Interval B, requiring repair within three calendar days. The research employs a qualitative Root Cause Analysis (RCA) approach, using Pareto Diagram, in-depth technician interviews, Fishbone Diagram mapping, and the 5 Why and Focus Group Discussion (FGD) techniques. Results indicate that connector corrosion is the dominant failure mode (50%), driven by the absence of corrosion inspection procedures in the Maintenance Planning Data (MPD), poor compliance with the Aircraft Maintenance Manual (AMM), and high humidity exposure in open hangars. The failure of the Twin Motor Actuator reflects systemic weaknesses in maintenance procedures, work culture, and operational environment, highlighting the need for strategic corrective actions to enhance component reliability and flight safety.
