Abstract

This study analyzes the recurrent failure of the Cabin Rate of Climb Indicator (P/N WL501RC1) on Boeing 737-800 aircraft maintained by PT. GMF AeroAsia Tbk. The research aims to identify the main causes of unscheduled removals and propose improvement strategies to enhance maintenance reliability. The study employs a qualitative method using the Root Cause Analysis (RCA) approach, supported by Pareto and Fishbone diagrams. Data were obtained through interviews, field observations, and technical documentation, including Component Removal Reports and Out-of-Tolerance Notifications. The results show that “Housing Leak” is the dominant failure mode, accounting for 46% of total cases between 2019 and 2024. The root causes were linked to human error in O-ring installation, outdated work procedures, reduced test bench accuracy, substandard repaired materials, and environmental exposure affecting component elasticity. Corrective and preventive actions include technical training, updated maintenance documentation, and stricter quality control on materials and testing equipment. These findings provide a practical basis for improving MRO performance and aircraft operational reliability.
