Introduction: Obesity during pregnancy is an increasingly common health problem and can increase the risk of various complications for both the mother and fetus. This condition is associated with a higher risk of gestational hypertension, preeclampsia, gestational diabetes mellitus, operative delivery, and fetal growth disorders. Therefore, comprehensive midwifery care is needed to monitor maternal health conditions and prevent complications during pregnancy. Objective: To describe the implementation of midwifery care for a pregnant woman with obesity at TPMB Mrs. “G”, Plaosan, Magetan. Methods: This report used a descriptive study with a case report design through the SOAP (Subjective, Objective, Assessment, Planning) approach in midwifery care management. Data were collected using primary and secondary data sources. Primary data were obtained through interviews, observations, and physical examinations, while secondary data were obtained from documentation such as the Maternal and Child Health (MCH) handbook and medical records. Data analysis was conducted descriptively based on subjective and objective findings, problem identification, care planning, implementation of interventions, and evaluation of care. Results and Discussion: The assessment of Mrs. W, a 31-year-old primigravida (G1P0A0) at 23 weeks of gestation, showed a Body Mass Index (BMI) of 31.64 kg/m², which was classified as obesity. The patient reported fatigue, back pain, and numbness in her hands. The care provided included education on balanced nutrition, recommendations for safe physical activity, monitoring of weight gain, and regular antenatal care examinations. After three months of follow-up care, the patient's condition improved as indicated by reduced physical complaints and weight gain within the recommended range. Conclusion: Comprehensive SOAP-based midwifery care can assist in monitoring maternal health, reducing the risk of pregnancy complications, and supporting the health of both mother and fetus.
