Abstract

Human Immunodeficiency Virus (HIV) attacks the immune system, causing a decline in immune function and potentially progressing to Acquired Immunodeficiency Syndrome (AIDS). Viral load testing is used to assess the amount of virus present in the blood and evaluate the effectiveness of treatment, while antiretroviral (ARV) therapy functions to suppress viral replication and improve immune function. This study aimed to analyze the relationship between viral load levels and adherence to ARV therapy among HIV patients at Puskesmas (Community Health Center) Gedongtengen, Yogyakarta. This study was an analytical observational study with a cross-sectional approach. Samples were selected using a total sampling technique, involving 34 respondents. Data were collected through questionnaires and medical records from December 2025 to January 2026. The results showed a significant relationship between viral load levels and adherence to ARV therapy (p = 0.000), indicating that patients with high adherence had undetectable or lower viral load levels compared to non-adherent patients. In conclusion, adherence to ARV therapy plays an important role in suppressing viral replication, maintaining low viral load levels, and improving treatment outcomes as well as patients’ quality of life.
