Abstract

Back pain is the most common physiological complaint experienced by pregnant women in the third trimester, resulting from postural changes, increased uterine load, and hormonal influences. This condition negatively affects quality of life and the mother's physical readiness for labor. Endorphin massage is a non-pharmacological intervention that works by stimulating endogenous endorphin secretion to suppress pain perception. This study aimed to identify the level of back pain before and after endorphin massage, and to analyze its effect on third-trimester pregnant women at RSUD Tombulilato. A quantitative approach with a pre-experimental one group pre-test post-test design was employed. A total of 20 respondents were selected using purposive sampling. Pain was measured using the Numeric Rating Scale (NRS) and data were analyzed using the Wilcoxon Signed Ranks Test. Results indicated that prior to intervention, 60.0% of respondents experienced moderate pain and 30.0% experienced severe pain. Following intervention, 70.0% reported mild pain and none remained in the severe category. Statistical analysis yielded Z = -4.243 with p = 0.000 (p < 0.05), confirming a significant effect of endorphin massage on reducing back pain. Endorphin massage is recommended as a routine non-pharmacological therapy in midwifery care.
