<![CDATA[Community Health Centers are first-level health service facilities that require information systems to support the delivery of information and services to the community. Tanjung Agung Muara Bungo Community Health Center does not yet have a digital medium that can be used to provide information about its profile, services, healthcare personnel, activities, and registration to the community in an easy and structured manner. This condition causes limited access to information, and the patient visit registration process is still carried out manually. Therefore, this study aims to design and develop a website-based Information System for Tanjung Agung Muara Bungo Community Health Center as a medium for delivering information and supporting patient registration administration. The research methods used were observation and interviews to obtain data regarding service conditions, the health center profile, healthcare personnel, service schedules, and system requirements. The system development uses the Waterfall method, which consists of the stages of requirements analysis, system design, implementation, testing, and maintenance. The system design uses Unified Modeling Language (UML), while the implementation uses PHP, HTML, CSS, JavaScript, Bootstrap, and MySQL. System testing was conducted using the Black Box Testing method by testing the main functions based on predetermined scenarios. The result of this study is a website-based Information System for Tanjung Agung Muara Bungo Community Health Center, which provides Home, About, Services, Doctor Profile, Online Registration, and Admin Login pages. The system is capable of presenting health center information in a structured manner and providing online registration to obtain queue numbers and monitor registration status. The admin panel can be used to manage profile, doctor, polyclinic, news, and registration data. The results of Black Box Testing on seven main features show that all tested functions operate according to the requirements.
