Abstract

Hypertension is one of the leading causes of chronic kidney disease (CKD) progression and is highly prevalent among patients undergoing hemodialysis. Appropriate antihypertensive therapy is essential to control blood pressure, delay disease progression, and reduce cardiovascular complications. Objective: This study aimed to evaluate the use of antihypertensive drugs in chronic kidney disease patients undergoing hemodialysis at Cempaka Putih Islamic Hospital, Central Jakarta, during January–March 2026. Methods: This study employed a descriptive retrospective design using medical record data of CKD patients undergoing hemodialysis. Patient characteristics, antihypertensive drug utilization, and the rationality of therapy were evaluated based on the parameters of appropriate patient, appropriate indication, appropriate drug, and appropriate dose according to the JNC VIII guideline. Results: The evaluation demonstrated that antihypertensive therapy in CKD patients was generally appropriate. The most frequently prescribed antihypertensive agents included candesartan, bisoprolol, amlodipine, clonidine, furosemide, Adalat Oros®, and spironolactone. Assessment of rational drug use showed 100% appropriate patient, 100% appropriate indication, 95.29% appropriate drug selection, and 91.76% appropriate dosage. Conclusion: Antihypertensive therapy among chronic kidney disease patients undergoing hemodialysis at Cempaka Putih Islamic Hospital was predominantly rational according to the JNC VIII guideline. Nevertheless, several cases of inappropriate drug selection and dosing were identified, indicating the need for continuous evaluation to optimize therapeutic outcomes and improve patient safety.
