Abstract

Plantar fasciitis is an inflammation of the plantar fascia. The physiotherapy problems identified included: (1) pain in the sole of the foot, (2) muscle spasm of the gastrocnemius muscle, (3) limited range of motion (ROM) in both ankle joints, (4) bilateral ankle muscle weakness, (5) decreased functional activity, and (6) tenderness in the calcaneal area. This study was conducted at Cahya Kawaluyan Hospital using a case study design involving a patient with bilateral plantar fasciitis et causa calcaneal spur who received physiotherapy interventions consisting of infrared (IR), Transcutaneous Electrical Nerve Stimulation (TENS), and exercise therapy, including free active exercise, calf raises, and active stretching. Data collection and analysis were performed using auto history. The instruments used focused on pain, muscle spasm, ROM, muscle strength, and functional activity. After four treatment sessions, the results were as follows: (1) pain decreased from 1.8 at T1 to 0 at T4; (2) muscle spasm decreased from 1 at T1 to 0 at T4; (3) ROM increased, with right ankle dorsiflexion improving from 15° at T1 to 25° at T4, right ankle eversion from 10° to 15°, left ankle dorsiflexion from 10° to 20°, and left ankle eversion from 5° to 15°; (4) muscle strength increased from grade 4 at T1 to grade 5 at T5; and (5) functional activity in daily living improved. The conclusion of this study is that TENS (Transcutaneous Electrical Nerve Stimulation), infrared (IR), and exercise therapy, consisting of free active exercise, calf raises, and active stretching, can reduce the problems associated with bilateral plantar fasciitis et causa calcaneal spur.
