Abstract

This qualitative study explores how healthcare workers perceive organizational justice and how these perceptions shape their job satisfaction in an Indonesian hospital. Using a reflexive thematic analysis guided by the Gioia methodology, this study examined lived experiences of fairness, leadership responsiveness, and trust in the hospital workplace through in-depth interviews with five healthcare workers across radiology, pharmacy, medicine, and nursing. The analysis produced five second-order themes organized under three aggregate dimensions: perceived imbalance between contribution and outcome, erosion of organizational trust, and psychological dissociation with a recovery-oriented orientation. Findings indicate that uneven workload distribution, limited leadership presence during periods of strain, and repeated unmet expectations progressively erode employees' trust, even as employees continue to perform their professional duties competently. Despite this erosion, participants consistently expressed hope for organizational repair rather than intentions to leave, suggesting a reservoir of goodwill that hospital management can still draw upon. The findings extend organizational justice theory by illustrating a process-oriented, rather than static, model of justice perception in a hierarchical, collectivist healthcare setting. This study discusses practical implications for hospital leadership, including transparent workload allocation and responsive crisis communication.
