Abstract

Purpose: This study addresses the limited integration between digital health innovation and community-based rehabilitation in rural stroke management by developing an integrated intervention combining the E-SiGap digital platform for early detection and eco-print-based rehabilitation for functional recovery and socioeconomic empowerment in Betoyoguci Village, Indonesia.Research Methodology: A structured five-stage community service model was implemented, consisting of socialization, training, technology implementation, mentoring, evaluation, and program sustainability. Data were collected through participant assessments and program performance evaluations. Results: The intervention improved community capacity in stroke prevention and rehabilitation. Health knowledge increased, with 91.3% of participants achieving high knowledge levels after the intervention. In addition, 97.6% successfully implemented home-based stroke rehabilitation, 96% of health cadres demonstrated competency in using the E-SiGap application, and 93% of the Stroke Hasta Karya Group successfully produced ecoprint products. Conclusions: The integrated digital and creative rehabilitation model provides an innovative community-based approach that supports stroke prevention, rehabilitation, and economic empowerment of stroke survivors. This program demonstrates how health technology and local resources can be combined to establish a sustainable rural stroke management model.Limitations: This pilot program was conducted in a single village, which limited its generalizability across different geographic and cultural settings.Contributions: This study contributes a scalable framework for translating clinical knowledge into community-driven digital and creative interventions, offering practical insights into sustainable rural healthcare empowerment.
