Abstract

Chronic rhinosinusitis with nasal polyps (CRSwNP) is commonly associated with type 2 inflammation, including increased eosinophils and elevated interleukins (IL)-4, IL-5, IL-9, IL-13, IL-25, and IL-33. Control of the disease may remain inadequate despite intranasal corticosteroids and endoscopic sinus surgery, creating a need for therapies directed at type 2 immune pathways. This systematic literature review summarizes the current knowledge base and major research directions concerning biologic treatment for CRSwNP. Literature was reviewed from PubMed, Google Scholar, Cochrane Library and other sources during May 10-15, 2026, using the terms “biologic” and “chronic rhinosinusitis with nasal polyps”; 127 e-journals were obtained. Across the reviewed literature, biologic therapy was associated with symptom improvement, a lower polyp burden, and less need for revision sinus surgery. Choice of treatment should be individualized according to defined clinical goals, while direct head-to-head trials are still needed to inform selection between agents. Long-term outcomes, combination strategies, and performance in routine clinical practice remain important research priorities. Biologic agents provide a developing therapeutic option for obtaining clinical benefit in CRSwNP.
