Abstract

The implementation of Electronic Medical Records (EMR) is mandatory for all health care facilities in Indonesia in accordance with Minister of Health Regulation No. 24 of 2022. However, primary health care facilities still face various challenges in its implementation. This study aims to analyze the challenges of implementing Electronic Medical Records using the Human Organization–Technology Fit (HOT-FIT) method at the Tajinan Community Health Center in Malang Regency. This study uses a descriptive design with a qualitative approach through a case study. Data collection was conducted through semi-structured interviews, direct observation, and documentation studies involving key informants and primary informants consisting of the head of medical records, medical record officers, doctors, nurses, pharmacists, and laboratory staff, who were selected purposively. Data analysis was performed using an interactive qualitative data analysis model. The results showed that the implementation of EMR provided benefits in the form of increased work efficiency, easier access to patient data, and improved medical record data security. However, there were still challenges in the human aspect in the form of variations in user competence and limited structured training. Organizational aspects include incomplete and unclear SOPs, suboptimal standard operating procedures, and unclear access rights regulations, while technological aspects include network instability and decreased system performance during peak service hours. Therefore, strengthening user capacity, organizational governance, and technological infrastructure is necessary to optimize the implementation of EMR in primary health care services.
