Abstract

Introduction: The increasing cultural diversity in healthcare highlights the need for transcultural nursing, yet its specific roles and mechanisms in improving patient-centered outcomes remain fragmented. The study aims to synthesize empirical evidence on the role and mechanisms underlying transcultural nursing in improving patient-centered outcomes. Methods: A Systematic review was conducted following the PRISMA 2020 guidelines. The literature was drawn from Scopus, PubMed, CINAHL, and Web of Science using predefined search terms related to transcultural nursing and patient-centered outcomes. Studies published between 2018 and 2026 are screened according to eligibility criteria, and methodological quality is assessed using the Joanna Briggs Institute's Critical Assessment Checklist. The data were synthesized using a narrative thematic approach. Results: Ten cross-sectional studies were included. Two key roles of transcultural nursing are identified: improving patient-centered care through culturally congruent individualized interventions, and strengthening therapeutic relationships through trust and effective communication. Five mechanisms support this role: cultural competency development, culturally sensitive communication, building therapeutic trust, culturally appropriate care planning and decision-making, and integration of family and organizational support. These mechanisms are associated with improved patient satisfaction, quality of care, safety, self-management, and quality of life. Conclusion: Transcultural nursing serves as a multidimensional clinical and relational framework that advances patient-centered outcomes through integrated professional, interpersonal, and contextual mechanisms. Strengthening transcultural nursing competencies and instilling culturally responsive systems in healthcare organizations is an important strategy for promoting fair, meaningful, and high-quality patient-centered care in an increasingly multicultural healthcare setting.
