The Bina Keluarga Balita (BKB) Program is a government initiative aimed at improving parents’ knowledge and skills, particularly mothers’, in childcare and the stimulation of growth and development among children under five. The success of the BKB Program is strongly influenced by the role of cadres in encouraging mothers’ participation. However, maternal participation in BKB activities in the working area of Aek Korsik Community Health Center has not yet reached the established target. This study aimed to determine the effect of cadre roles on mothers’ participation in the Bina Keluarga Balita (BKB) Program in the working area of Aek Korsik Community Health Center, North Labuhanbatu Regency, in 2026. This quantitative study used a cross-sectional design. The study population consisted of 1,476 mothers with children under five years of age. A total of 94 mothers were selected using cluster random sampling. Data were analyzed using univariate analysis, bivariate analysis with the chi-square test, and multivariate analysis using logistic regression. The bivariate analysis showed significant associations between mothers’ participation in the BKB Program and the cadre roles as mobilizers (p = 0.000), educators (p = 0.005), and facilitators (p = 0.004). In contrast, the role of cadres as recorders and reporters was not significantly associated with mothers’ participation (p = 0.736). Multivariate analysis indicated that the cadre role as a mobilizer was the most dominant factor associated with mothers’ participation (p = 0.000; Exp(B) = 5.528). The roles of cadres as mobilizers, educators, and facilitators significantly influence mothers’ participation in the Bina Keluarga Balita Program in the working area of Aek Korsik Community Health Center, North Labuhanbatu Regency. Mothers are encouraged to actively participate in BKB activities and maintain effective communication with cadres.
