Hospital outpatient services require an effective health administration system to ensure service quality, patient safety, and organizational efficiency. Integrating local wisdom into health administration management may strengthen communication, patient-centered care, and service acceptance within local communities. This paper aims to describe the mapping of health administration systems and identify managerial problems based on local wisdom in outpatient services. This study employs a qualitative method with a descriptive approach to identify the health administration system and managerial issues—based on local wisdom—regarding outpatient services at Dr. Hafiz Hospital, Cianjur. The study results indicate that the health administration system for outpatient services at Dr. Hafiz Hospital, Cianjur, comprises five key components: patient registration, medical record management, the hospital information system, human resource management, and service monitoring and evaluation. The study also identified several managerial issues, including ineffective communication, suboptimal inter-unit coordination, inconsistent implementation of standard operating procedures (SOP), underutilization of the hospital information system, and suboptimal integration of local wisdom values into the delivery of outpatient services. Mapping the health administration system and identifying managerial issues provide a strategic foundation for strengthening outpatient service governance through enhanced coordination, information system optimization, human resource competency development, consistent implementation of standard operating procedures (SOP), and the integration of local wisdom values to improve the quality of healthcare services.
