Abstract

Perineal wounds require care to support the healing process and prevent complications. Binahong leaves contain various bioactive compounds. Developing a binahong leaf extract-infused sanitary pad represents an innovative delivery system for applying natural ingredients to the perineum. Objective: To characterize and evaluate the safety of a binahong leaf extract-based sanitary pad for postpartum perineal wound care, assessing flavonoid content, antibacterial and anti-inflammatory activity, pH, chlorine content, and irritation potential. Methods: This was a product development study utilizing a laboratory evaluation approach. 1,000 g of binahong leaves were extracted using 70% ethanol via maceration for 72 hours (3 × 24 hours) and concentrated through vacuum distillation. The liquid extract was used to coat tissue paper, which was then placed inside the sanitary pad. Evaluations covered flavonoid levels, antibacterial activity against Staphylococcus aureus, anti-inflammatory activity, pH, chlorine content, and irritation. Results: The binahong leaf extract had a flavonoid content of 548.56 mg QE/100 g. Antibacterial testing showed no inhibition zones at extract concentrations of 25%, 50%, and 100% against S. aureus, whereas the positive control (1% chloramphenicol) produced inhibition zones of 21.20 mm and 21.50 mm. Anti-inflammatory activity of the extract at a 0.1% concentration showed 20.367% inhibition. The pad had a pH of 5, tested negative for chlorine, and showed no irritation in tests involving four panelists. Conclusion: The binahong leaf extract-based sanitary pad contains flavonoids, exhibits anti-inflammatory activity, has a pH of 5, is free of chlorine, and causes no irritation. These results provide a preliminary basis for the further development of binahong leaf extract-based sanitary pads as a product to support perineal wound care.
