Abstract

The mass deactivation of Health Insurance Premium Assistance Recipient (PBI JK) membership based on the National Single Social and Economic Data (DTSEN) since 2025 risks cutting off access to health services for poor and vulnerable groups in Purwakarta Regency, making the reactivation mechanism critical for the sustainability of Universal Health Coverage (UHC). This study analyzes the implementation of that reactivation policy using Hill and Hupe's (2002) policy implementation theory, covering seven indicators: policy characteristics, policy formation, policy transfer process, implementer response, horizontal inter-organizational relations, target group response, and policy environment. This research employs a qualitative method with a case study approach; data were collected through in-depth interviews with 12 purposively selected informants, observation, and documentation, then analyzed using Miles and Huberman's (1994) interactive model. Results show that despite a clear tiered legal basis and functioning formal bureaucratic channels, actual reactivation capacity lags far behind the scale of deactivation: only 923 participants (1.16%) were reactivated out of 79,606 deactivated participants (January 2025-March 2026), constrained by limited village-level authority and weak information transfer to target groups.
