Abstract

The development experience since Indonesian independence generally shows that welfare is still out of reach. It shows that there is a need to replace the basic ideology of development operational from material to human beings. The operational vision need to be changed from creating a welfare nation to creating a morally nation that has high competency since the latter measurement comprise more, materially or non-materially.In this writing, writer stated that the absolute requirement for the development success of developing countries, including Indonesia, is strategy orientation changes from outward looking to inward looking; from basic operational ideology of material to human and humanity ideology.

Abstract

Based on past experience as well as implementation paradigms embedded in the five-year plan 2004-2009, there is a very small likelihood that it can be successfully implemented. In the past before the crises of 1997, for a long time economic growth has been on average 6.8% per year. But that did not prevent open unemployment from rising. The plan target of reducing open unemployment to 5.1% in 2009 from about 9.7% in 2005 with 6.6% yearly average of economic growth appears very unrealistic. This unrealism is the more so because the plan as it stands embodies paradoxes in its various agendas and programs of action resulting from the operation of Arrows impossibility theorem. While the plan aims at increasing productive employment, its assumption of human being as a resource is a passive, order receiving rather than an active, innovative and risk taking one.More fundamentally, even if the plan target of economic growth is achieved, it, being on the physical plane, will fail, as it has in the past, to translate the spiritual and corporeal values of Belief in One God and Just and Enlightened Humanity contained in the 1945 constitution. Indonesian society will move further and further away from the cherished ideals of the founding fathers of the Republic with dire implications on its future prosperity and even of its existence. It has been proposed that the operational objective of maximizing economic growth be replaced by maximizing human capability which is more suited to accommodate the multiplicity of objectives in the 1945 constitution with the different levels of physical, corporeal and spiritual dimensions.

Abstract

Oral potentially malignant disorders have been associated with a betel quid chewing habit. To date, betel quid chewing has a major social and cultural role in the society of Samosir Island, North Sumatra, Indonesia. The purpose of the present study was to evaluate the occurrence of oral potentially malignant disorders associated with the habit of betel quid chewing in the society of Samosir Island, North Sumatra, Indonesia. This case-control study was conducted on all betel quid chewers in the working area of the Ambarita Public Health Centre in Samosir Regency, North Sumatra, Indonesia. All subjects were examined clinically for the presence of any oral lesions and interviewed for their betel quid chewing habit. Chi-square and Fisher’s exact tests were used to analyze the relationship between variables. Among the 51 subjects recruited in the study, 28 subjects suffered from oral potentially malignant disorders who had oral potentially malignant disorders such as submucous fibrosis and/or leukoplakia (the case group) whereas 23 subjects showed no clinically detectable oral potentially malignant disorders (the control group). An increased risk of oral potentially malignant disorders was associated with the habit of chewing areca nut and tobacco (OR=1.600; p=0.542), the duration of betel quid chewing more than 25 years (OR=4.379; p=0.023), and the frequency of betel quid chewing more than 6 times/day (OR=4.800; p=0.021). In conclusion, oral potentially malignant disorders were associated with chewing betel quid habit in the society of Samosir Island, North Sumatra, Indonesia.

Abstract

Oral health is one of the aspects that should be concerned during the pregnancy. During this time, the hormonal changes may create so me changes in different part of body such as oral and dental. For examples, gingivitis, pyogenic granuloma, dental caries and tooth erosion. These conditions can be prevented if enough and timely care of oral and dental parts i s taken. The aim of this stud y was to know woman’s knowledge, attitude, and practice on oral and dental health during pregnancy. This was a cross sectional study among 100 pregnant women who visited Poliklinik Ibu Hamil RSUD Dr. Pirngadi Medan. Data collected by questionnaire. The res ults showed that only 5% of the pregnant women knew that pregnancy had an effect on their teeth and gums and only 3% of the women knew that oral disease could affect on their fetal health and growth. More than half didn’t know that tetracycline could affec t their baby’s teeth. Only 4% of the women received oral health education, and only 8% of the women reported to have visited dentist during pregnancy. Pregnancy experience was the major sources of oral health education and that all women were brushing their teeth daily, while 37% of the women believed that tooth brushing would reduce gum diseases. In addition, respondents never received an advice to see a dentist during pregnancy from obgyne. In conclusion, most of the participants in this study had in adequate oral health knowledge although majority of them were brushing their teeth daily. Likewise, most of the participants had not received oral health education, and only few of them reported to have dental visits. Therefore, researchers and health program p lanners should give increased attention to the oral health needs and behaviors of pregnant women.

Abstract

The oral lichenoid reaction is to be regarded as a reaction pattern within the oral mucous directed towards a specific agents in patients predisposed to develop such a reaction. During the last decade, it has been discussed that dental amalgam plays etiological role in the development of lichenoid reaction in oral mucous. Removal of amalgam fillings has led to healing or clinical improvement of a majority of existing lichenoid lesions. We reported a case of oral lichenoid reaction associated to amalgam restoration. A 53 years old woman presented with a sore mouth and ulceration affecting the gingival and buccal mucous in the region of maxillary right first molar filled with amalgam. Patient had no medical history and medication. After replacing amalgam restoration with resin composite and applied of topical corticosteroid, the lesion clinically disappeared. In conclusion, this case suggested that replacement of amalgam restoration with the other material and applied topically steroid be effective in the resolution of the oral lichenoid lesion related to amalgam restoration.

Abstract

Xerostomia merupakan sensasi subjektif mulut kering yang dapat menyebabkan berbagai masalah di rongga mulut danmempengaruhi kualitas hidup seseorang. Xerostomia sering terjadi sebagai efek samping penggunaan obat dan salah satuobat yang dapat menyebabkan xerostomia adalah obat bronkodilator yang digunakan pasien penyakit paru obstruktifkronik (PPOK). Tujuan penelitian untuk mengetahui hubungan penggunaan obat bronkodilator terhadap terjadinyaxerostomia pada pasien penyakit paru obstruktif kronik (PPOK) berdasarkan jenis dan lama pemberian obat. Jenispenelitian ini adalah survei analitik dengan pendekatan cross sectional melibatkan 97 subjek (75 orang laki-laki dan 22orang perempuan) yang merupakan pasien PPOK di RSU Dr. Pirngadi Medan. Penelitian ini dilakukan denganmewawancarai subjek menggunakan alat bantu kuesioner. Data diproses secara komputerisasi dan analisis menggunakanuji Chi square. Hasil penelitian menunjukkan pasien PPOK yang mengalami xerostomia sebanyak 63 orang (64,95%).Persentase xerostomia paling tinggi terdapat pada pengguna kombinasi obat (agonis beta 2 dan antikolinergik) danberdasarkan lama pemberian obat paling tinggi terdapat pada pengguna obat selama 1-5 tahun. Sebagai kesimpulan,ditemukan adanya hubungan yang signifikan antara jenis obat dan lama pemberian obat bronkodilator yang digunakanpasien PPOK terhadap terjadinya xerostomia. Penggunaan kombinasi obat dan dengan durasi yang semakin lama akansemakin meningkatkan resiko terjadinya xerostomia.

Abstract

Oral candidiasis is one of the most common opportunistic infections in HIV/AIDS patients. It serves as important markers of HIV infection, viral load, and CD4 cells count in the blood and predict disease progression to AIDS. The development of oral candidiasis in HIV/AIDS patients associated by imbalances between Candida and impaired host immune defenses that caused by decreased of CD4 cell counts and the increased of plasma HIV-viral load. Since the introduction of antiretroviral therapy combination, commonly known as Highly Active Antiretroviral Therapy (HAART), it has been observed that certain oral lesions, such as oral candidiasis as declined. The aim of this paper is to review the mechanism of combination antiretroviral therapy influenced the frequency of oral candidiasis in HIV/AIDS patient. We conclude that combination antiretroviral therapy generally reduced the frequency and severity of oral candidiasis in HIV/AIDS patient.

Abstract

Introduction: Oral Squamous Cell carcinomas (OSCC) are still common, and one of the causes is the human papillomavirus (HPV). OSCC-associated HPV is often detected at an advanced stage due to the low knowledge of dentists about early detection. A good level of knowledge among dentists will help reduce the incidence of OSCC- associated HPV cases. This study aims to determine the level of knowledge of general dentists in Medan City regarding the role of HPV in OSCC, clinical aspects, prognosis, preventive measures for OSCC- associated HPV, and the availability of HPV vaccines in Indonesia. Method: This research is a descriptive study with a cross-sectional approach; the number of respondents is 99 general dentists in Medan City. The data was collected using an online questionnaire. Data analysis was carried out descriptively and calculated in terms of frequency and percentage through SPSS. Result: The results showed that among the 99 respondents, the level of knowledge about the role of HPV in OSCC was good (98%). The level of knowledge regarding the clinical aspects of OSCC-associated HPV is good (64.6%). The level of knowledge regarding the prognosis of OSCC-associated HPV is adequate (70.7%). The level of knowledge regarding OSCC- associated HPV preventive action is good (79.8%). The level of knowledge regarding the availability of the HPV vaccine in Indonesia is good (67.7%). Conclusion: Based on the results of the study, the level of knowledge of general dentists in Medan City about Oral Squamous Cell Carcinoma (OSCC) related to Human Papillomavirus (HPV) is good (66.7%).
