Abstract

The IFNG +874 A/T polymorphism is a genetic variation in the first intron of the IFN-γ gene that affects interferon-gamma production through an NF-κB binding mechanism. The T allele is associated with high IFN-γ production, while the A allele is associated with low production. The AA genotype is considered a risk factor for active tuberculosis. This review aims to analyze the role of the IFNG +874 A/T polymorphism in tuberculosis immunopathogenesis, emphasizing differences between active and latent TB and other influencing variables. Literature was searched in PubMed, Scopus, Google Scholar, ScienceDirect, and Cochrane Library (January–June 2026) using keywords related to IFNG, TB, and LTBI. Results showed that global meta-analyses confirmed the protective effect of the T allele (OR 0.60–0.78), but the Asian subgroup showed inconsistencies. A three-group study in Brazil proved that the AA genotype was associated with active TB compared to LTBI. This polymorphism interacts with TLR2, VDR, IL4, comorbidities, Mtb lineage, epigenetic factors, and the environment. In Indonesia, only two studies with conflicting results and no latent cohort were available, indicating a significant research gap. This review concludes that the IFNG +874 A/T polymorphism is an important genetic determinant in tuberculosis immunopathogenesis, but further studies with a three-group design and control for confounding variables are needed in various Indonesian populations.
