Abstract

Hemodialysis cannulation action will respond to discomfort due to stimulation of a large needle puncture. The puncture stimulates sensory nerve fibers which cause pain. This study aims to determine the differences in the effectiveness of cold compresses and deep breath relaxation techniques to reduce patient pain during hemodialysis cannulation (femoral access inlet) in the Hemodialysis Unit of the Cilacap Hospital. This type of research is a quasy experimental study with a static group comparison design of 12 people who received cold compresses and 12 people who received deep breath relaxation techniques that were taken using the total sampling method. Data analysis using t-test. The results showed that there was a significant difference in the pain scale of patients during cannulation (inlet femoral access) hemodialysis before and after being given cold compresses (pv = 0.001, α = 0.05), there was a significant difference in the pain scale of patient during cannulation (inlet femoral access) hemodialysis before and after being a deep breathing relactation technique (pv = 0.002, α = 0.05), there was no significant difference between the patient’s pain scale during cannulation (femoral access inlet) hemodialysis after being given cold compresses and after being given deep breathing relaxation techniques (pv = 0,081, a = 0,05).

Abstract

Hemodialisis merupakan terapi pengganti ginjal pada penderita gagal ginjal kronik di Indonesia. Pasien yang menjalani terapi hemodialisis akan mengalami perubahan hampir seluruh aspek kehidupannya meliputi fisik, psikologis, sosial, ekonomi dan spiritual. Penelitian ini bertujan untuk mengetahui hubungan mekanisme koping dan dukungan keluarga dengan tingkat stres pada pasien gagal ginjal kronik yang menjalani hemodialisis di RSU Nirmala Purbalingga. Jenis penelitian ini merupakan studi korelasi dengan rancangan cross sectional, pengambilan sampel dengan metode total sampling, adapun instrumen penelitian menggunakan kuisioner dan uji analisis menggunakanuji Contingency coefficient. Hasil penelitian pasien hemodialisis di RSU Nirmala Purbalingga tahun 2019 memiliki mekanisme koping adaptif (51,0%), dukungan keluarga tinggi (59,2%) dan tingkat stres ringan (59,2%). Ada hubungan yang signifikansi antara mekanisme koping dengan tingkat stres pada pasien GGK yang menjalani hemodialisis di RSU Nirmala Purbalingga (ρ value = 0,000 < α 0,05) dan ada hubungan yang signifikansi antara dukungan keluarga dengan tingkat stres pada pasien GGK yang menjalani hemodialisis di RSU Nirmala Purbalingga (ρ value= 0,001 < α = 0,05).

Abstract

Adequacy of hemodialysis is the adequacy of the number of hemodialysis processes which is characterized by patients feeling better and comfortable. Hemodialysis access can be done with an Arterio-Venous (AV) shunt will increase blood flow rapidly. Another access to get central venous access can use the femoral AV. The use of hemodialysis access can affect adequacy. The purpose of this study was to determine differences in the adequacy of hemodialysis in patients with AV shunt and AV Femoral access. This research uses quantitative research with comparative descriptive method. Time approach using cross sectional. A sample of 32 people were divided into 2 groups, namely the AV shunt access group and the AV femoral access group. The sampling technique used a purposive sampling technique in accordance with the inclusion and exclusion criteria. Data analysis used independent t-test. The average urea level in hemodialysis patients with AV shunt access was 74.72% and AV femoral access was 67.05%.There was a significant difference in the adequacy of hemodialysis in patients with AV shunt and AV femoral access (pv = 0.013). Cilacap General Hospital is advised to improve its services by opening an AV shunt installation facility and providing education to Chronic Renal Failure (CRF) patients so that patients are motivated to install an AV shunt.
