Abstract

Adherence to treatment is a big challenge for thalassemia patients. Iron chelation is one of treatment that must be carried out by thalassemia patients for preventing complications due to iron accumulation in the body's organs. Most of the deaths in thalassemia patients are caused by complications due to accumulation of iron from blood transfusions. Adherence to iron chelation treatment is a protective factor for thalassemia mortality. The objective of the study is to analyze the reIationship between knowledge and social support with adherence score to iron cheIation treatment in thaIassemia patients. This study uses a quantitative method with a cross-sectionalI approach, the data obtained were 84 respondents. Primary data was collected by interview using a questionnaire. The statistical test used was Pearson correlation. The resuIts of bivariate anaIysis show that age knowledge about thalassemia (p=0,000;r=0,491-0,554), knowIedge about  iron cheIation (p=0,000;r=0,554), famiIy support (p=0,000;r=0,570), and health workers support (p=0,000; r=0,468). It can be concluded that knowledge about thaIassemia, knowIedge about iron cheIation, famiIy support, heaIth workers support, have a significant relationship with adherence score to taking iron chelation in thaIassemia patient. Keywords: adherence, iron chelation, thalassemia

Abstract

Objective: This study is to analyze the factors associated with adherence score to iron chelation treatment in thalassemia patients at RSUD Tidar Magelang in 2019. Methods: Quantitative methods are used in this research.An analytical study was carried out with a cross-sectional design with total sampling. Subjects were thalassemia patients who had routine transfusions at RSUD Tidar in 2019.A list of patients was obtained from the transfusion visit book at Thalassemia’s Room, RSUD Tidar. Primary data was collected by interview using a standardized questionnaire. Interview with parents is done for respondents below 16 years old. The statistical test used was Pearson correlation and Independent sample t-test. Results: Of the 90 patients on the list, 84 were successfully interviewed. From that, 54 (64,4%) patients had an adherence score of iron chelation medication below the average. Duration of taking medication from patients was 1-16 years with an average of 7.5 years Results of bivariate analysis show that age (p=0,086;r=-0,189),sex (p=0,737;r=0,190),knowledge about thalassemia (p=0,000;r=0,491-0,554),knowledge about iron chelation (p=0,000;r=0,554), family support (p=0,000;r=0,570), health workers support (p=0,000; r=0,468), duration of taking medicine(p=0,040;r=-0,224),side effects (p=0,000;r=-0,518),access to hospital (p=0,403) and type of iron chelation (p=0,023). Conclusions: It can be concluded that knowledge about thalassemia, knowledge about iron chelation, family support, health workers support, side effects of medicine, type of iron chelation, duration of taking medicine have a significant relationship with adherence score to taking iron chelation in thalassemia patient. Update on knowledge related to thalassemia and iron chelation is needed for patients and their families. Attention and motivation from families and health workers are needed, especially for patients who are bored with treatment.

Abstract

Objective: Up to 1.5 years of the pandemic, Gunungkidul Regency reported approximately 12 types of clusters, one of which was the Takziah Cluster in Dusun Panggang I, Giriharjo, Panggang. On March 27, 2021, the Gunungkidul Regency Health Office received a report of 6 people from a family who were symptomatic with positive RT-PCR results. It is known that the family head of the family died with probable status. Epidemiological investigations are carried out to determine the magnitude of the problem, contact history with the suspect, and prevent the spread of the case. Method: This research uses a descriptive approach. Cases are people in Dusun Panggang I who have an epidemiological relationship with people who take part in takziah activities with positive RT-PCR results. Data were collected by interview and secondary data obtained from the Puskesmas and the Health Office. Results: Cases started from probable who were buried without a health protocol. A total of 40 confirmed cases were found through mass screening of 630 residents on March 27- April 26, 2021 (Attack Rate=6.50%) with 2 deaths (Case Fatality Rate=4.88%). The majority of cases were women (73.17%) and aged >60 years (41.46%). The most common symptoms experienced were fever (43.34%), cough (34.15%), and fatigue (29.3%). From the interviews, family and neighbors gather to cook and mourn without maintaining distance, and wearing masks are not correct. Conclusions: There has been a transmission of COVID-19 in Dusun Panggang I caused by a takziah event. The lack of proper use of masks and crowding without keeping a distance is one of the risk factors for transmission during takziah. Takziah by following health protocols is mandatory during the COVID-19 pandemic. Education and clear enforcement of regulations from local governments are very much needed.

Abstract

Purpose: This study aims to asses the needs for identifying health challenges and the requirements for health promotion training in Mulyodadi Hamlet, Bantul. Method: The focus group discussion (FGD) method was employed in this study, involving dukuh and cadres, to obtain in depth information. Results: The FGD findings underscored the key health challenges in Mulyodadi, which include the prevalence of both communicable and non-communicable diseases, a lack of public awareness about the importance of a healthy lifestyle, and unequal access to health information within the community. It was also found that dukuh and cadres did not carry out widespread health promotion for the general public. Their educational efforts, often delivered through posyandu (integrated health service posts), were primarily targeted at parents of infants and toddlers, thus limiting their health promotion knowledge and skills to this context. Conclusion: To optimize their role in health promotion within the community, dukuh and cadres require health communication training, specifically elevator pitch skills training. This training will equip them with the ability to deliver concise, compelling, and effective health promotion messages, thereby addressing the identified health challenges more flexibly in terms of time and place.
