Abstract

Herpes zoster (HZ) is an acute vesicular eruption caused by reactivation of latent varicella-zoster virus (VVZ) in sensory ganglia previously exposed to primary varicella infection. The incidence of HZ increases with age but many case reports show that herpes zoster can also occur in adolescents and even in children. The purpose of this case report is to implement a holistic and comprehensive family doctor service by identifying risk factors, clinical problems, and patient management based on evidence-based medicine with a patient-centered and family approach. This study is a case report study. Primary data were obtained through history taking (autoanamnesis from the patient and alloanamnesis from the patient's family), physical examination and home visits. Secondary data were obtained from the patient's medical record. Assessment is based on a holistic diagnosis from the beginning, process, and end of the study qualitatively and quantitatively. Patient Ms. K, 7 years old, has been diagnosed with uncomplicated herpes zoster, has concerns about his health condition, the patient feels inferior and is afraid that the disease cannot be cured and is afraid of transmitting it to other family members. The patient hopes that the complaints disappear completely and do not recur, so they can be comfortable with their activities as usual. The patient's limited knowledge of the illness and an unfavorable lifestyle are factors that cause problems that occur in patients and their families. The diagnosis of herpes zoster, in this case, was in accordance with several theories and a critical review of recent studies. Management has been given in accordance with the management guidelines. There is good progress in patients starting from decreased clinical symptoms to changes in behavior after an intervention based on evidence-based medicine that is family-approach and patient-centered.

Abstract

Varicella or chicken pox is an acute and highly contagious disease caused by infection with the Varicella Zoster virus (VZV). Apply the principles of family medicine services and carry out holistic management based on a framework for solving patient problems with a patient-centered, family focused and community-oriented approach based on Evidence Based Medicine. This study analysis is a case report. Primary data is obtained through history taking, physical examination and home visits to complete family data, psychosocial and environmental data. Secondary data was obtained from patient medical records at the Community Health Center. The assessment is carried out based on a holistic diagnosis of the beginning, process and end of the study quantitatively and qualitatively. Male patient Mr. C 27 years-old came with complaints that he had tingles on his face, neck, body, back, arms, and leg for two days ago accompanied by itching. Elastic in the form of vesicles the size of billions, round shape, well-defined, with an erythematosus base, distributed regionally. Clinically and on physical examination the patient was diagnosed with Varicella (ICD 10 B01.9). In this case, diagnosis and treatment have been carried out according to theory and related journals. After the intervention was carried out, there was a reduction in clinical symptoms and an increase in patient and family knowledge. Holistic management can improve the knowledge, attitudes and behavior of patients and families in maintaining health.

Abstract

Metabolic syndrome is a cluster of risk factors including hypertension, dyslipidemia, central obesity, and insulin resistance that increase the risk of cardiovascular disease. Elderly individuals are particularly vulnerable due to physiological aging, reduced physical activity, and unhealthy dietary habits. This case report presents a comprehensive management of a 60-year-old female patient diagnosed with metabolic syndrome, consisting of hyperuricemia, hypercholesterolemia, and grade I hypertension. The patient presented to a primary care facility with complaints of shoulder pain and headache, and a sedentary lifestyle. Physical and laboratory examinations confirmed obesity class I (BMI 29.1), total cholesterol of 263 mg/dL, and uric acid level of 7.2 mg/dL. A multidisciplinary treatment approach was applied, combining pharmacological therapy (allopurinol, simvastatin, ibuprofen, and vitamin B12) with non-pharmacological interventions, including nutritional education, lifestyle modification, and aerobic exercise. Follow-up evaluations at two weeks and one month showed symptom relief, improved laboratory values, and enhanced patient knowledge and compliance. The integrated family medicine approach proved effective in improving the patient’s quality of life and awareness of long-term metabolic syndrome control.

Abstract

Gout Arthritis is a chronic progressive non-communicable disease caused by monosodium urate (MSU) crystal deposits. The prevalence of gout in Indonesia is the highest among other Asian countries. Diabetes and Hypertension are non-communicable diseases that occur in the world. Diabetes mellitus (DM) is a metabolic disease caused by impaired insulin secretion or insulin function. Type 2 diabetes mellitus is the leading cause of death in the world. The prevalence of type 2 DM sufferers according to the 2018 Riskesdas is 8.5%. Hypertension is a silent killer disease, according to the 2018 Riskesdas the prevalence of hypertension was 34.1%, an increase compared to 2013 which was 25.8%. Hypertension is a condition when blood vessel pressure is more than 140/90 mmHg. According to WHO 1.28 billion adults aged 30-79 years worldwide suffer from Hypertension. The diagnosis of Gout arthritis, Diabetes mellitus, and Hypertension is made based on clinical and supporting findings. A holistic approach based on family physicians was implemented through drug and non-drug interventions, including nutrition education, physical exercise, and family counseling in three home visits. Evaluation showed a decrease in uric acid levels, blood sugar, and improvement in blood pressure, as well as an increase in understanding and changes in patient behavior related to disease management. Patients began to implement a healthy lifestyle, such as regular exercise and food recording. An evidence-based medicine approach with family support has been shown to be effective in improving disease control and patient quality of life.

Abstract

Anemia remains a significant health concern, particularly among pregnant women, despite existing intervention efforts. Its prevalence is influenced by several risk factors, including inadequate iron intake, leading to iron deficiency anemia. A holistic approach is necessary to manage anemia effectively during pregnancy. This case report aims to identify the risk factors and clinical condition of a pregnant patient in order to provide comprehensive management that is patient-centered, family-focused, and community-oriented. The subject of this case is Mrs. A, a 29-year-old woman who was five weeks pregnant and presented with fatigue and dizziness for approximately one month. The diagnosis of iron deficiency anemia was made, and her condition was classified as functional class II. Both internal and external risk factors contributed to her clinical presentation. Management of her condition was carried out according to current theory and evidence-based medicine. The intervention focused not only on treating clinical symptoms but also on empowering the patient and her family to improve health behavior. A holistic approach in the treatment of disease is essential as it helps reduce risk factors by increasing patient awareness and encouraging sustainable lifestyle changes.
