Abstract

Background: To enhance the quality of nursing service at hospital, it needs learning process (education and training) using clinical skills facility. It gives ideal place for the development of nurse’s confident before entering the real world of work. Problem based learning approach is able to use simulation learning method or Practice Based Simulation Model can enhance new nurse’s confidence. Objective: This study aimed at investigating the literature for of the practice based simulation that may be expected to enhance new nurse’s self confidenceMethod: This research used Literature Review. Source of the research was taken from some database. Searching was conducted in December 2018 through database of PubMed of 3016 articles, Google Scholar of 13.5000 articles, Science Direct of 3466 articles, Pro Quest of 256 articles using combination of key words “self confidence” and practice based simulation+new graduates nursing. From all database, there were only 7 which fulfilled inclusion criteria.Result: based on the result of literature review from 7 articles, it was found that applying simulation method to new nurse can enhance self confidence. New nurse would be more independent, able to make decision and conduct nursing care and be more calm while working.Conclusion: the aim of this literature review obtains the result that application of practice based simulation can increase self confidence of nurse.

Abstract

ABSTRACT Circulatory disorders are one type of emergency condition that is often found in the IGD so that it requires fast and appropriate assistance. Shock is a clinical condition due to inadequate tissue perfusion. The ABC protocol is a systematic and structured approach that is very important in handling emergency patients, including shock patients. The method used is descriptive observational. The population is all IGD nurses and a sample of 30 nurses who work on morning and afternoon shifts. Data collection using observation sheets according to standard operating procedures for the management of shock patients compiled based on the American Heart Association (AHA) and PPNI guidelines, and was carried out for 2 (two) weeks. The protocol was carried out well with the results of checking airway patency 56.6%, Performing the head-tilt chin-lift maneuver correctly 43.3%, Providing oxygen correctly 63.3%, Monitoring oxygen saturation 36.6, Checking pulse and initial blood pressure 53.3%, IV access, fluid administration and re-evaluation 46.4%. The results of the study showed that most nurses implemented the ABC protocol well, but there were several factors that influenced the ABC protocol not being carried out properly, namely: high workload, limited resources and nurses not working according to standard operating procedures. Another thing that influenced the ABC protocol was the lack of understanding of nurses regarding standard operating procedures. The evaluation showed that the implementation of the ABC protocol by IGD nurses was good, but not yet fully optimal, especially in terms of airway and follow-up monitoring. Continuous training, routine clinical audits and direct surveys are needed, and increased awareness of the importance of early stabilization according to the protocol. Keywords: Airway, Breathing, Circulation, Shock  ABSTRAK Gangguan sirkulasi adalah salah satu jenis kondisi gawat darurat yang sering dijumpai di IGD sehingga membutuhkan pertolongan yang cepat dan tepat. Syok adalah suatu keadaan klinis akibat perfusi jaringan yang tidak adekuat. Protokol ABC merupakan pendekatan sistematis dan terstruktur yang sangat penting dalam penanganan pasien gawat darurat tidak terkecuali pada pasien syok. Metode yang dilakukan adalah deskriptif observasional. Populasi seluruh perawat IGD dan sampel sebanyak 30 perawat yang bertugas pada sift pagi dan sore. Pengumpulan data menggunakan lembar observasi sesuai standar prosedur operasional penatalaksanaan pasien syok yang susun berdasarkan pedoman American Heart Association (AHA) dan PPNI, dan dilakukan selama 2 (dua) minggu. Protokol dilakukan  baik dengan hasil:memeriksa patensi jalan napas 56,6%, melakukan manuver head-tilt chin-lift secara tepat 43.3%, memberikan oksigen dengan benar 63,3%, melakukan pemantauan saturasi oksigen 36,6, pemeriksaan nadi dan tekanan darah awal 53,3%, Akses IV,pemberian cairan dan evaluasi ulang 46,4%. Hasil penelitian menunjukkan sebagian besar perawat melaksanakan protokol ABC dengan baik, Namun ada beberapa faktor yang mempengaruhi protokol ABC tidak dilakukan dengan tepat yaitu: beban kerja yang tinggi, keterbatasan sumber daya dan perawat tidak bekerja sesuai  standar prosedur operasional, Hal lain yang mempengaruhi protokol ABC adalah kurangnya pemahaman perawat terhadap standar prosedur operasional. Evaluasi menunjukkan bahwa penerapan protokol ABC  oleh perawat IGD sudah baik, namun belum sepenuhnya optimal terutama dalam hal airway dan monitoring lanjutan. Diperlukan pelatihan berkelanjutan, audit klinis dan survey langsung secara rutin, dan peningkatan kesadaran pentingnya stabilisasi awal sesuai protokol. Kata Kunci: Airway, Breathing, Circulation, Syok
