Abstract

I Infection Prevention and Control Nurse (IPCN) has an essential role in preventing and controlling infections in hospitals. IPCN's role has been developed to optimize the prevention and monitoring of infection in hospitals. Some countries such as Korea, the United Kingdom, Ireland have developed graduate program nurse control specialists by obtained needs for particular science that can deal with hospital conditions, specifically related to the prevention of infection and care of infectious patients. This case study aims to identify and analyze IPCN's optimization of IPCN in conducting infection prevention and control in a government hospital in Jakarta, which is also a national referral center hospital. Data collection was done by observing the daily activity of the IPCN, reviewing IPCN's role by focus group discussion with nine out of ten IPCNs, and interviewing the nurse committees and director of nursing. The results obtained indicate that the implementation of the functions and roles of IPCN cannot be carried out maximally. This is because, in addition to serving as IPCN, IPCN was also involved in the activities carried out by the unit as well as the implementation of wide surveillance hospitals by the Hospital Infection Control Committee (HICC). It is recommended to optimize the implementation of the functions and roles of the IPCN through the establishment of IPCN competency guidelines aimed at improving IPCN competency standards now and in the future, as well as guiding the determination of IPCN's duties and responsibilities to be more maximal in preventing and controlling infections. Abstract

Fracture is a loss of bone continuity, either total or partial, caused by physical trauma, angular force, force, condition of the bone, and soft tissue; which can be treated through surgery, with one of the problems namely postoperative pain. This study aimed to determine the effectiveness of cold packs and breathing relaxation to treat post-Open Reduction Internal Fixation (ORIF) pain at Mitra Sejati General Hospital Medan. This study applied a one group pretest-posttest design, involving 37 patients. In the pre- and post-treatment phases, pain levels were measured, then a difference analysis was performed using a paired samples t-test. After giving cold packs and deep breathing relaxation techniques, the results showed a decrease in pain levels. The number of patients experiencing mild pain increased to 20 people (54.1%). The p value of the results of data analysis was 0.03 (there was a difference in pain levels between before and after treatment). It was concluded that giving cold packs and deep breathing relaxation is effective for dealing with post-ORIF pain, so cold packs and deep breathing relaxation can be used as independent nursing actions to reduce pain intensity.Keywords: fracture surgery; painful; cold packs; deep breath relaxation ABSTRAK Fraktur merupakan hilangnya kontinuitas tulang, baik yang bersifat total atau sebagian yang disebabkan oleh trauma fisik, kekuatan sudut, tenaga, keadaan tulang, dan jaringan lunak; yang bisa ditangani melalui operasi, dengan salah satu masalah yakni nyeri pasca operasi. Penelitian ini bertujuan untuk mengetahui efektifitas cold pack dan relaksasi nafas untuk mengatasi nyeri pasca Open Reduction Internal Fixation (ORIF) di Rumah Sakit Umum Mitra Sejati Medan. Penelitian ini menerapkan rancangan one group pretest-posttest, yang melibatkan 37 pasien. Pada fase sebelum dan sesudah perlakuan dilakukan pengukuran tingkat nyeri, lalu dilakukan analisis perbedaan menggunakan paired samples t-test. Setelah pemberian cold pack dan teknik relaksasi nafas dalam, didapatkan hasil penurunan tingkat nyeri. Jumlah pasien yang mengalami nyeri ringan bertambah menjadi 20 orang (54,1%). Nilai p dari hasil analisis data adalah 0,03 (ada perbedaan tingkat nyeri antara sebelum dan sesudah perlakuan). Disimpulkan bahwa pemberian cold pack dan relaksasi nafas dalam efekif untuk mengatasi nyeri pasca ORIF, maka cold pack dan relaksasi nafas dalam dapat dijadikan sebagai tindakan mandiri keperawatan untuk menurunkan intensitas nyeri.Kata kunci: operasi fraktur; nyeri; cold pack; relaksasi nafas dalam

Abstract

Pain during arteriovenous fistula (Cimino) puncture in chronic kidney failure patients undergoing regular hemodialysis can cause discomfort, anxiety, depression, and low self-esteem. This study aims to determine the effectiveness of using ice cold pack therapy and slow deep breathing on pain during puncture in patients with arteriovenous fistula (Cimino). The research design used was a quasi-experimental research design with two groups pre-post test conducted at Efarina Etaham Berastagi Hospital. One group was given experimental treatment, namely ice cold pack therapy and slow deep breathing, while the other group as the control group was only given slow deep breathing treatment. Data analysis was carried out to see the relationship between the independent and dependent variables using the independent samples t-test at a significance level of α = 0.05. The independent samples t-test results obtained a p value> 0.05, namely 0.104, meaning that statistically there was no significant difference in the average pain scale between patients given ice cold pack therapy and slow deep breathing with patients only given slow deep breathing. Nurses are advised to continue to carry out slow deep breathing therapy and ice cold packs to overcome pain during puncture in patients with arteriovenous fistula (Cimino).
