Abstract

Breast cancer is a major health problem for women. This cancer is most often diagnosed and the leading cause of cancer deaths in women worldwide. Breast cancer in young women is associated with late diagnosis, aggressive biological characteristics, and poor prognosis. The objective of the study is to determine the prevalence and characteristics of young age breast cancer in Haji Adam Malik General Hospital Medan in 2013-2017. The study design was observational descriptive design with a cross-sectional approach. The study samples were medical records of young women diagnosed with breast cancer who were treated at Haji Adam Malik General Hospital Medan. The prevalence of breast cancer in young age women in Haji Adam Malik General Hospital Medan in 2013-2017 was 234 patients (15.2%). In this study, the samples that fulfilled the inclusion criteria were 153, with the majority characteristics of patients were 35-39 years old, stage III, histopathological type of invasive carcinoma of no special type, grade II, and clinical subtype of triple-negative. The prevalence of young age breast cancer is lower than old age breast cancer. Young women tend to be diagnosed with breast cancer in advanced stage and intermediate grade with aggressive biological characteristics.

Abstract

Background: Breast cancer is one of the cancers with the highest prevalence in Indonesia. According to GLOBOCAN data for 2020, the number of breast cancer cases reached 68,858 (16.6%) cases out of a total of 396,914 new cancer cases in Indonesia. There are many factors that can trigger cancer with different clinical manifestations and subtypes from one event to another, so researchers are interested in researching the profile of breast cancer patients with the HER2 subtype at H. Adam Malik General Hospital in 2020-2021.Objective: In the present study, we aim to determine the clinicopathological profile of Her2 positive (non-luminal) breast cancer subtype patients at Haji Adam Malik General Hospital from 2020-2021.Methods: The method used in this study is descriptive quantitative with total sampling technique. The data samples was obtained from medical records of breast cancer patient with her2 positive (non-luminal) subtype which was then sorted according to the availability of characteristic needed for this study. Results: majority of breast cancer patients with her2 positive (non-luminal) subtype at H Adam Malik General Hospital in 2020-2021 was within the age group of > 50 years, with all married status. the BMI of the majority of patients (68.6%) was normal weight, all respondent admit there was no family history diagnosed with cancer (100%), majority tumor size (52,9%) is >5 cm, with most patients having cancer metastasized to 1-3 lymph nodes (54,9%), distant metastases only in 39,2% of the total sample observed, majority (35,3%) histopathological degree is in grade 1 and grade 3 , the histopathological classification of the majority (86,3%) was IDC type. Conclusion: Overall there is diversity of frequencies within the observed clinicopathological characteristics.

Abstract

Introduction: Phyllodes tumor (PT) is infrequent among breast neoplasms, representing less than 1% of all breast neoplasms, with 10% to 30% of them being malignant. The occurrence rate of malignant phyllodes tumor (MPT) is about 2.1 cases per million people.Case Presentation: We report the case of a 36-year-old female who had a significant mass in her right breast. Macroscopically, the cut surface was grayish-white with cystic areas containing a hemorrhagic mass; the tumor occupied nearly the entire breast with indistinct borders. The tumor measured 26 x 16 x 11 cm. Microscopically, the tumor exhibited a solid structure with ductal proliferation and hypercellular stroma forming a leaf-like appearance. Proliferation of fibrous connective tissue with dysplastic cell morphology and atypical mitoses was found in 10/10 high-power fields (HPFs). Malignant heterologous components included bone matrix with osteoclast-like giant cells and chondroid components. Immunohistochemical Ki-67 staining was positive in more than 20% of the cell nuclei. Conclusion: The tumor is large with unclear infiltration boundaries and contains malignant heterologous components. Immunohistochemical Ki-67 staining was positive in more than 20% of the cell nuclei, confirming this as a case of malignant phyllodes tumor. This case is notable due to its rarity, frequent recurrence, and the challenges in establishing a diagnosis and treatment regimen.
