Abstract

Objective: The aim of this article was to reports our experiences PCNL on ADPKD patients. Case(s) Presentation: We report the case of a 38-year-old male with autosomal dominant polycystic kidney disease associated with bilateral staghorn diseases. We performed a supine PCNL on left kidney. Discussion: ADPKD may arise sporadically, the developmental abnormality that resulting in multiple cysts, hypertension, hematuria, polyuria, plank pain, and are prone to reccurent urinary tract infection and renal stones. ADPKD is one most commonly genetics disorder than frequently results in End Stage Renal Disease (ESRD). Population of ADPKD has a higher risk to have a nephrolithiasis, thus it may hastened the onset of ESRD. Open surgeries was one of treatment of choices of ADPKD with staghorn stones, but considering of high rates of morbidity. Recently there’s consistent switching the trend on minimal invasive treatment such as ESWL and PCNL. Conclusion: PCNL may considered as an effective and safe procedure in managing nephrolithiasis in ADPKD, further studies with larger sample and longer periods observation is needed to confirmed role of PCNL in preserving kidney on ADPKD with staghorn kidney.

Abstract

Objective: To determine Type-2 diabetic patients and its characteristics to determined risk of Erectile dysfunction (ED) and the role of EHS score as more practical tools to screening ED among T2DM patients. Material & Methods: The cross-sectional observational study was carried out at the Internal medicine Unit of Manambai Abdulkadir and Dompu district Hospital, out of 45 patients were included on this study. The data were collected from June 01 – August 01 2017. A structural questioner was used to collect the data and was analyzed using SPSS 2.0. person analysis correlation and logistic regression were used to find the Odds Ratio (OR). Results: We found that 38 out of 45 (84.4%) patients (mean age 57.2 ± 7.1) have Erectile dysfunction, 26 patients uncontrolled diabetes, 5 (11.1%) treated as CHF, 4 (8.9%) CKD, and 7 (15.6%) neuropathy diabetic. There is high correlation between random glucose level and IIEF-5 score (r=0.5, p=0.01). The overall odd ratio of ED in this studies was 4.3 (95% CI: 0.73 to 25.1) for uncontrolled diabetes, 2.5 (95% CI: 0.1 to 51.1) for Treated CHF, 2 (95% CI: 0.01 to 41.6) for CKD, and it was 1.2 (95% CI: 0.1 to 11.5) for neuropathy diabetics. Conclusion: From this study, we found that most diabetic patients have ED, and there is high correlation between random glucose level and simplified IIEF-5 score, EHS performed similar result on diagnoses ED compared to IIEF-5.

Abstract

Objective: This study aims to compare 4 years of experience of IC and TUUC in the same period and among similar experienced surgeons. Material & Methods: Between January 2016 and August 2019, 44 radical cystectomies were performed, but 4 patients were excluded due to incomplete data or who underwent neo-bladder procedures. The primary endpoint was 30 days of complication rate and intraoperative complications. Bowel movement, ambulation, and length of stay (LOS) postoperatively were followed-up over a period of 30-day postoperatively. Results: 12 male patients underwent TUUC and 24 male patients IC, while only 4 female patients underwent IC. The mean of LOS of IC was 12.72 8.6 and 10.08 3.5 for TUUC; there were no significant differences between arms. However, TUUC had lower intra-operatively bleeding (779.17 441.15 ml) compared to IC (1328.57 810.40 ml). There was no difference in early complications between arms. Conclusion: Our results suggest that TUU with UC diversion may be used as a viable option of urinary diversion in radical cystectomy. This technique provides similar safety both surgically and oncologically.

Abstract

Antibiotic resistance is currently an emergent global challenge. Urinary tract infection (UTI), one of the most commonly reported infections, are is becoming a difficult case to treat considering the increasing prevalence of antimicrobial resistance (AMR). The objective of this study was to assess the impact of a hospital antibiotic stewardship program on AMR in managing UTI at a tertiary referral hospital in Yogyakarta, Indonesia, while considering the absence of the program in lower-tier referral hospitals. A retrospective cross-sectional study was conducted from January 2017 to December 2020, classified into pediatric and adult samples. Urine samples were collected and cultured from all patients with UTI hospitalized in the Dr. Sardjito General Hospital, Yogyakarta. The UTI causative bacteria and antibiotic susceptibilities were investigated in the comparison of the first 2 years (2017-2018, prior to the hospital antibiotic stewardship program) and the last 2 years (2019-2020, following the implementation of the hospital antibiotic stewardship program). The isolates from 717 adult urine samples were cultured. Escherichia coli (39.1%), Acitenobacter baumanii (9.3%), and Pseudomonas aeruginosa (8.5%) were identified as the most common bacteria prior to the hospital antibiotic stewardship program. Extended-spectrum β-lactamase-producing E. coli and Burkholderia cepacia were still increasing in the following the implementation of the hospital antibiotic stewardship program. Our study indicated that the stewardship program does not exhibit a significant change during the first two years considering the absence of the program in lower-tier referral hospitals.

Abstract

In Indonesia, the proportion of non-muscle bladder cancer is low compared to other countries. Doxorubicin is currently one of options for adjuvant therapy for nonmuscle invasive bladder (NMIBC) that underwent transurethral resection of bladder tumor (TURBT). This study is aimed to evaluate the oncological outcome of doxorubicin intravesical. Eight consecutive T1 bladder patients who underwent TURBT with complete tumor resection, and received intravesical chemotherapy were enrolled in this study. All the clinical data were gathered retrospectively. One of eight patients did not respond to intravesical doxorubicin instillation. Patients were recorded with univocal, low grade and female patients. In conclusion, doxorubicin intravesical may be one of options. Despite a small number of samples were enrolled. This preliminary data showed promising results on intravesical doxorubicin instillation.
