Abstract

Kondisi pro-inflamasi memiliki peran penting dalam perkembangan, invasi, dan metastasis kanker kolorektal (KKR). Beberapa penelitian mengungkapkan berbagai gen pro-inflamasi dalam karsinogenesis KKR, termasuk cyclooxygenase-2 (COX2) dan Chitinase-3-like protein 1 (CHI3L1). Penelitian ini bertujuan untuk mengidentifikasi polimorfisme gen COX-2 dan CHI3L1 pada pasien KKR di Bali. Data berasal dari 39 sampel pasien KKR, termasuk usia, jenis kelamin, diagnosis patologi, grade, dan lokasi tumor. DNA diekstraksi dari sampel FFPE. Genotipe SNP COX-2 (rs20417) dan CHI3L1 (rs4950928) diidentifikasi menggunakan real time quantitative PCR menggunakan primer spesifik (rhAmpTM SNP Genotyping System kit, IDT Technologies). Semua data dianalisis dengan SPSS versi 21. Hasil penelitian menunjukkan bahwa usia rata-rata adalah 59,82±11,35 tahun, lebih tinggi pada kelompok usia di atas 50 tahun sebanyak 31 sampel (79,5%). Sebagian besar sampel berjenis kelamin laki-laki sebanyak 24 sampel (61,5%) serta adenokarsinoma sebagai diagnosis patologis terbanyak sebanyak 32 sampel (82,1%). Selain itu, sebagian besar sampel menunjukkan low grade sebanyak 33 sampel (74,4%) dan lokasi tumor pada sisi kiri sebanyak 24 sampel (61,5%). Analisis hubungan SNP COX-2 (rs 20417) serta SNP CHI3L1 (rs4950928) dengan diagnosis patologi, grade dan lokasi menunjukkan nilai p>0,05. Secara keseluruhan, dapat disimpulkan bahwa kasus CRC lebih tinggi pada usia di atas 50 tahun, jenis kelamin laki-laki, low grade dan tumor sisi kiri. Dalam penelitian ini tidak ditemukan hubungan yang signifikan antara genotipe COX-2 dan CHI3L1 dengan diagnosis patologi, grade, dan lokasi tumor.

Abstract

Introduction: Pulmonary empyema is an infectious disease with mortality and morbidity rates ranging between 3% and 33%. It occurs in approximately 8.9% of patients with tuberculosis (TB), and the associated mortality rate is approximately 20%. If not treated promptly following diagnosis, empyema may result in a poor prognosis. Therefore, this case report presented a patient with pulmonary empyema caused by a possible TB infection. Case: A 57-year-old female presented with a two-week history of coughing up yellow phlegm and a three-day history of fever. The patient reported an untreated dental cavity in the right lower tooth for the past three weeks. Physical examination revealed decreased vesicular breath sounds in the right hemithorax’s fifth to sixth intercostal space (ICS). Chest X-ray examination suggested pneumonia and right pleural effusion. Laboratory examinations revealed leukocytosis, thrombocytosis, and an increased neutrophil-to-lymphocyte ratio (NLR). The adenosine deaminase (ADA) level was 89 U/L. Ultrasonography (USG) of the right hemithorax confirmed right pleural effusion. A pleural puncture was performed, and a thoracic drainage was inserted. The procedure yielded approximately 1,700 cc of cloudy brown pus. The patient was diagnosed with right pulmonary empyema caused by tuberculous pleurisy. She received antibiotic therapy and a four-drug fixed-dose combination (FDC) of anti-TB therapy. After nine days of treatment, the patient’s symptoms showed improvement. Conclusion: Empyema is a complex disease with diverse etiologies and multifactorial pathogenesis. Early detection and prompt treatment are essential to minimize the risk of further complications.

Abstract

Introduction: Despite the effective global mass vaccination programs, which included booster shots, the emergence of new SARS-CoV-2 strains threatens to undo the enormous success achieved thus far in stopping its spread. This study aimed to assess the demographics of COVID-19 patients from February to May 2021. Methods: Data were collected using secondary data from the medical records of Klungkung Regional General Hospital, Klungkung, at the time range according to the study period. Statistical Package for the Social Sciences (SPSS) version 25.0 for Windows and Microsoft Excel 2016 were used for the statistical analysis. All quantitative data, such as age, length of stay, and neutrophil-to-lymphocyte ratio (NLR), were estimated using central location measures (mean range). Proportions were used to describe qualitative or category variables. Results: The demographic description of COVID-19 patients at Klungkung Regional General Hospital, Klungkung, found an average age of 51.94 years old. More patients were found to be male (55.4%) and did not have comorbidities (55.3%). Fourteen patients (14.4%) were gravid, and 37 patients (17.8%) were treated in the Intensive Care Unit (ICU). A total of 61 patients (29.3%) had desaturase with an NLR (mean) of 6.97 (51% increase). The average length of stay was 10.16 days, with the result of 24 patients dying (11.5%). Conclusion: Patients with COVID-19 were found to have milder symptoms with less use of the ICU, not all of them experienced decreased saturation, and the case fatality rate was 11.5%.
