Abstract

Introduction: Hemodialysis (HD) therapy is the most common therapy for patients with End Stage Renal Disease (ESRD). Insomnia prevalent among dialysis patients which affects their quality of life and death. Although hemodialysis therapy has been proven to prolong survival, the action of hemodialysis therapy has complications that cause symptoms, one of which is sleep disturbance. Complaints of sleep disorders that are often encountered in dialysis units are experienced by 50-80% of dialysis patients. Of the various sleep disorders, insomnia is one of the symptoms that is often experienced by ESRD patients. Methods: A literature review comparing several literatures through the search of accredited journal sites such as PubMed, Elsevier, ResearchGate, and Neliti with the keywords management insomnia in hemodialysis patients in the 2015-2020 period. Research Results: Management of insomnia is reviewed with the aim of providing guidance to encourage treatment. In the six journals reviewed, education on sleep hygiene, CBT-I, relaxation therapy, massage therapy, and drug administration (with caution) improving the quality of sleep for hemodialysis patients. Conclusions: Pharmacological and non-pharmacological management to overcome insomnia in hemodialysis patients can be given by considering the factors that influence it. Management is advised to use a nonpharmacological approach first. The administration of pharmacological therapy is recommended only for short-term use, used with caution in hemodialysis patients.

Abstract

Terapi hemodialisa (HD) merupakan terapi yang paling banyak dilakukan pasien End Stage Renal Disease (ESRD). Meski terapi hemodialisa telah terbukti dapat memperpanjang kelangsungan hidup, tindakan terapi hemodialisa memiliki komplikasi yang memunculkan gejala-gejala salah satunya adalah gangguan tidur. Keluhan gangguan tidur yang sering ditemui dalam unit dialisis dialami oleh 50-80% pasien dialisis. Dari berbagai gangguan tidur, insomnia merupakan salah satu gejala yang sering dialami pasien ESRD. Sebuah kajian literatur yang membandingkan beberapa literatur melalui penelusuran situs jurnal terakreditasi seperti PubMed, Elsevier, ResearchGate, dan Neliti dengan kata kunci management insomnia in hemodialysis patient dalam kurun waktu 2015-2020. Manajemen insomnia ditinjau dengan tujuan memberikan panduan untuk mendorong pengobatan. Dalam enam jurnal yang dikaji, edukasi sleep hygiene, CBT-I, terapi relaksasi, terapi pijat, dan pemberian obat-obatan (dengan perhatian) memperbaiki kualitas tidur pasien hemodialisa. Manajemen farmakologis dan non-farmakologis untuk mengatasi insomnia pada pasien hemodialisis dapat diberikan dengan mempertimbangkan faktor-faktor yang mempengaruhinya. Manajemen disarankan untuk menggunakan pendekatan nonfarmakologis terlebih dahulu. Pemberian terapi farmakologis direkomendasikan hanya untuk penggunaan jangka pendek, digunakan dengan hati-hati pada pasien hemodialisis.

Abstract

Uremic pruritus was a distressing symptom frequently experienced by patients undergoing hemodialysis, characterized by widespread daily itching that was symmetrical, bilateral, without dermatomal patterns or primary skin lesions. The symptoms could be generalized or localized, particularly on the back, abdomen, and arms. This systematic review aimed to identify complementary therapy methods and their frequency of administration in managing pruritus among patients with Chronic Kidney Disease-associated Pruritus (CKD-aP) undergoing hemodialysis. The study used a systematic literature review design with a PICOST approach. The included studies were randomized controlled trials and quasi-experimental studies published between 2014 and 2024. The literature search was conducted through PubMed, Springer Link, ScienceDirect, SAGE Journals, Taylor & Francis, and Semantic Scholar. Article selection included duplicate removal using Mendeley, followed by title, abstract, and full-text screening based on inclusion and exclusion criteria. A total of 13 articles met the eligibility criteria. The results showed that acupressure, acupuncture, foot reflexology massage, clove oil, violet oil, virgin coconut oil, peppermint oil, EPA supplementation, and dill oil were effective in reducing pruritus among CKD-aP patients. The most frequently used interventions were peppermint oil and virgin coconut oil through topical application or massage. Complementary therapies had the potential to serve as effective adjunctive treatments, although their effectiveness varied and some studies had methodological limitations.
