Abstract

Epilepsi adalah gangguan otak yang ditandai dengan kecenderungan terjadinya kejang atau bangkitan epileptik. Epilepsi yang ditimbulkan oleh tumor otak disebut epilepsi terkait tumor otak. Patofisiologi yang mendasarinya masih belum banyak dipahami tetapi telah terbukti bersifat multifaktorial. Tujuan penulisan artikel ini adalah untuk melaporkan kasus dari seorang pasien perempuan berusia 50 tahun datang dengan keluhan utama kejang disertai benjolan di mata kanan yang terus membesar selama 6 bulan terakhir. Pemeriksaan fisik menunjukkan proptosis, gangguan visus dan papilatrofi pada mata kanan, paresis nervus kranialis III, IV, dan VI dekstra, lesi nervus kranialis V sensoris cabang I dan II dekstra, refleks primitif glabela positif, dan gangguan kognitif berat dengan ketergantungan ringan-sedang. Hasil radiologi menunjukkan suspek tumor sinonasal dekstra dengan infitrasi serebral. Hasil elektroensefalografi menunjukkan gambaran abnormal dengan frontal bilateral epileptiform discharges (L>R) disertai focal slowing regio frontalis dan frontal intermittent rhythmic delta activity (FIRDA) yang konsisten dengan klinis focal seizures disorder dapat sesuai dengan epilepsi lobus frontalis. Pasien mendapatkan terapi steroid deksametason intravena, fenitoin, asam valproat, asam folat, kalium aspartat, dan haloperidol. Pasien dipulangkan setelah perawatan hari kesepuluh dalam kondisi bebas kejang selama tujuh hari. Kondisi tumor pada kasus telah mencapai stadium lanjut, sebagaimana ditunjukkan oleh invasi tumor ke struktur tulang di sekitarnya dan struktur intrakranial sehingga memicu terjadinya epilepsi. Epilepsi dapat terkontrol dengan terapi steroid intravena serta obat anti epilepsi kombinasi.  KATA KUNCI: epilepsi; tumor otak; tumor sinonasal.

Abstract

Epilepsy is recognized as the commonest serious neurological disorder in the world. Women with epilepsy  experience several gender-related physical and social problems. They constitute high obstetric risk because of reduced fertility, risk of seizures during pregnancy, and complications of pregnancy. Hormonal and other factors can alter the pharmacokinetics of antiepileptic drugs during pregnancy and puerperium. Antenatal exposure to antiepileptic drugs, particularly at higher dosage and in polytherapy, increases the risk of fetal malformation. Recent reports raise the possibility of selective developmental language deficits and neurocognitive deficits with antenatal exposure to antiepileptic drugs. There are concerns regarding the effect of traces of antiepileptic drugs that pass to the infant during breast-feeding. The pre conception management is the cornerstone for epilepsy care in Women with epilepsy. A careful reappraisal of each case should ascertain the diagnosis, the need for continued antiepileptic drugs therapy, selection of appropriate antiepileptic drugs, optimization of the dosage, and prescription of folic acid. During pregnancy, the fetal status needs to be monitored with estimation of serum a-feto-protein and ultrasound screening for malformations. The dosage of antiepileptic drugs can be adjusted according to clinical requirement and blood levels of antiepileptic drugs. Several institutions recommend oral vitamin K toward the end of pregnancy when enzymeinducing.Key Words : epilepsy, pregnancy, management, complication

Abstract

The library is a professional work environment where library personnel are engaged in static and repetitive activities, which pose a risk of musculoskeletal disorders (MSDs) that can affect staff well-being and service quality. This study addresses the issue of occupational health among library staff from an ergonomic perspective, focusing on the risk of musculoskeletal complaints resulting from static workloads and suboptimal working postures. The study aims to describe the characteristics of library personnel at the Islamic University of Indonesia (UII) Library, identify the level of MSD complaints, analyze the relationship between ergonomic risk factors (sitting duration, working hours, and body mass index) and MSDs, and examine the implications for human resource management policies in libraries to improve information service quality. Using a descriptive quantitative approach, a case study method, and the Nordic Body Map questionnaire administered to 32 library staff, data were analyzed using Spearman correlation tests. The results show that working hours (p=0.009; r=0.456), sitting duration (p=0.044; r=0.359), and body mass index (p=0.043; r=0.360) have significant correlations with MSD complaints, which are most commonly reported in the upper neck, lower back, and right shoulder areas. These findings highlight the urgency for library management to integrate ergonomic principles into human resource management policies, including workspace design, task scheduling, and occupational health programs. Optimizing the physical health of library personnel not only helps reduce MSDs but also enhances productivity, job satisfaction, and the overall quality of information services in modern libraries.
