Abstract

Pregnancy is a time of significant physiological, psychological, and emotional changes in a woman's body. As the fetus develops, the mother's body undergoes changes that affect various bodily functions, one of which is increased urinary frequency. This case study was conducted on 10 third-trimester pregnant women who experienced frequent urination and its impact on their sleep. The study aims to identify triggering factors and assess the impact on sleep quality in third-trimester pregnant women. Sleep disturbances caused by frequent waking to urinate can negatively affect the physical and mental well-being of pregnant women. The study involved several third-trimester pregnant women who reported frequent nighttime urination. The results show that the majority of the participants woke up 2-5 times each night to urinate, leading to fragmented sleep and reduced sleep duration and efficiency. These sleep disturbances are associated with excessive daytime sleepiness, fatigue, and a decline in the quality of life of the pregnant women. Additionally, the findings also indicate that physical factors such as the enlarged uterus pressing on the bladder and hormonal changes play a significant role in increasing urinary frequency. The obstetric care management in this case study recommends interventions such as education on fluid intake regulation, Kegel exercises, and relaxation techniques like yoga to reduce sleep disturbances. It is hoped that this case study can provide insights for healthcare professionals to offer more effective management to improve the sleep quality and overall well-being of pregnant women.

Abstract

This case study aims to describe the comprehensive observation of labor progress in a multiparous woman, Ny. D, who underwent spontaneous vaginal delivery with oxytocin induction due to postdate pregnancy. Labor observation was carried out intensively using systematic monitoring of uterine contractions, cervical dilation, fetal heart rate, maternal vital signs, and partograph recording. The case presentation includes detailed findings during the latent and active phases, decision-making during induction titration, and management according to safe delivery standards. Discussion highlights the alignment and variations between case management and current evidence-based guidelines, particularly regarding the timing of induction, assessment of fetal well-being, and documentation accuracy. The conclusion emphasizes that structured and continuous labor observation significantly contributes to safe maternal and neonatal outcomes. Recommendations include optimizing decision-making protocols, strengthening partograph utilization, and enhancing clinical readiness in monitoring induced labor

Abstract

Premature Rupture of Membranes (PROM) is one of the obstetric complications that often causes morbidity and mortality in both mothers and infants. This condition occurs when the amniotic membranes rupture before the onset of labor contractions or before the gestational age reaches 37 weeks. PROM increases the risk of intrauterine infection, preterm labor, neonatal sepsis, and asphyxia if not properly managed. This case study aims to describe the application of Evidence Based Practice (EBP) in midwifery care for a woman in labor with PROM at Al-Fatah General Hospital Ambon. The case involved Mrs. D, a 22-year-old woman, G2P1A0, at 34 weeks of gestation, who was admitted to the hospital with complaints of watery discharge from the birth canal accompanied by lower abdominal pain and blood-stained mucus. Examination revealed cervical dilation of 7 cm, absence of membranes, fetal head at Hodge III, and a fetal heart rate of 145 beats per minute. The diagnosis was established as intrapartum stage I active phase with preterm premature rupture of membranes (PPROM). Midwifery management included close observation of labor progress, administration of Ringer’s Lactate infusion, and prophylactic intravenous Cefotaxime according to WHO guidelines. After four hours without progress, collaboration was carried out with an obstetrician, and a cesarean section (SC) was performed due to inadequate labor progression and risk of intrauterine infection. The implementation of EBP in this case proved to enhance maternal and neonatal safety through rapid, rational, and collaborative clinical decision-making. This evidence-based approach reflects Al-Fatah General Hospital Ambon’s commitment to providing safe, effective, and high-quality midwifery care.
