Abstract

Patient safety incident reporting is part of the patient safety programme. At the hospital level, this reporting is to find out the causes of patient safety incidents and find the root causes to anticipate that the incident will not happen again. According to the National Reporting and Learning System (NRLS) report, from August 2021 to July 2022, there were 2,410,311 reports of patient safety incidents in the UK. According to Malaysian Ministry of Health (MoH) data, there were 151,225 reports of patient safety incidents in 2021. The purpose of this scoping review is to evaluate the current literature on barriers to reporting patient safety incidents and summarise evidence relating to differences in barriers to reporting patient safety incidents in Indonesian and overseas hospitals. This research uses the scoping review method. Scoping review is a method used to identify literature comprehensively through various sources with various research methods and related to the research topic. The results showed that the barriers in reporting patient safety incidents are Management Support, Lack of Education and Training, Patient Safety Knowledge, Safety Motivation, Workload, Coworker Support. Barriers to reporting patient safety incidents, both in hospitals in Indonesia and abroad, have similar problems.

Abstract

ABSTRACT Patient safety is a system designed to improve security in health services, one of which is through reporting patient safety incidents. According to Milgram, factors that influence compliance come from internal factors (individuals), namely personal responsibility and external factors (organizations), namely location status, legitimacy of authority figures (legitimacy of authority figures), status of authority figures, support from colleagues and proximity of authority figures. To analyze organizational factors in compliance with reporting Patient Safety Incidents (IKP) at Kudus Regency Hospital. This study is a type of qualitative research using an in-depth interview method. The subjects involved in this study were 22 as main informants and 7 as triangulation informants. Data collection in the study was carried out using interview and documentation methods. Analysis of research data was carried out using qualitative descriptive analysis. The research shows that compliance with patient safety incident reporting (IKP) in hospital "X" is influenced by various organizational factors. Although the hospital has a strong commitment to patient safety, there are still obstacles in the reporting system. Legitimacy and the status of authority figures play an important role in transparency and staff compliance with reporting procedures. Compliance in IKP reporting does not only depend on the policies implemented, but also on organizational factors such as location status, legitimacy of authority figures, status of authority figures, support from peers, and proximity to authority figures.  Keywords: Organizational Factors, Reporting Compliance, Patient Safety Incidents, Hospitals.  ABSTRAK Keselamatan pasien adalah sistem yang dirancang untuk meningkatkan keamanan dalam pelayanan kesehatan, salah satunya melalui pelaporan insiden keselamatan pasien. Menurut Millgram, faktor yang mempengaruhi kepatuhan (Compliance) berasal dari factor internal (individu) yaitu tanggung jawab personal dan factor eksternal (organisasi) yaitu status lokasi, legitimasi figur otoritas (keabsahan figur otoritas), status figur otoritas, dukungan sesama rekan dan kedekatan figur otoritas. Menganalisis faktor organisasi yang mempengaruhi kepatuhan pelaporan insiden keselamatan pasien di Rumah Sakit Kabupaten Kudus. Penelitian ini merupakan jenis penelitian kualitatif dengan menggunakan metode wawancara mendalam (indepth interview). Subjek yang terlibat dalam penelitian ini yakni 22 sebagai informan utama dan 7 sebagai informan triagulasi. Pengumpulan data dalam penelitian dilakukan dengan menggunkana metode wawancara dan dokumentasi. Analisis data hasil penelitian dilakukan dengan menggunakan analisis deskriptif kualitatif. Hasil penelitian menunjukkan bahwa kepatuhan pelaporan insiden keselamatan pasien (IKP) di Rumah Sakit Kabupaten Kudus dipengaruhi oleh berbagai faktor organisasi. Terdapat kendala dalam sistem pelaporan meskipun Rumah sakit memiliki komitmen tinggi terhadap keselamatan pasien. Legitimasi dan status figur otoritas memainkan peran penting dalam transparansi serta kepatuhan staf terhadap prosedur pelaporan.Kepatuhan dalam pelaporan IKP bukan hanya bergantung pada kebijakan yang diterapkan, tetapi juga pada faktor organisasi seperti status lokasi, legitimasi figur otoritas, status figur otoritas, dukungan sesama rekan, kedekatan figur otoritas Kata Kunci: Faktor Organisasi, Kepatuhan Pelaporan, Insiden Keselamatan Pasien, Rumah Sakit.
