Abstract

Background: Root canal treatment can fail due to several causes. One of them is root canal calcifications. Purpose: To explain root canal retreatment technique in premolar with calcification. Case(s): A 65 years old female patient came to universitas airlangga dental hospital with a chief complaint of pain on tooth number 34 and 35. Tooth number 34 and 35 had an old restoration with secondary caries. Both teeth did not respond to vitality test. Both teeth were tender on percussion and bite test. Surrounding soft tissue was normal. Radiograph image reveals inadequate root canal obturations on both teeth and a periapical lesion on tooth 35. Case Management: Old restoration and root canal filling material was removed, then root canal retreatment was performed. Subsequently the tooth was restored with fiber post and zirconia crown. Discussion: old restoration has to be removed gently in order to prevent further damage to tooth structure. Root canal filling material can be removed with heated instruments, solvent, and rotary instruments. After root canal filling material removal, the remaining root canals need to be thoroughly shaped, cleaned, and sealed. Conclusion: root canal retreatment on premolar with an impediment can be successfully performed with appropriate instruments, materials, and technique.

Abstract

Background: Dental hard tissue loss renders a tooth restorable or unrestorable. The treatment ranges from endodontic treatment to dental implant. Sometimes patients in need to restore their oral function and esthetics cannot afford those treatments due to a limited financial capability and time availability. Purpose: This case report presents a minimal invasive, single appointment, quick, and affordable alternative treatment to restore morphology, function, and esthetics. Case: A 40 years old male patient suffered enamel-dentin-pulp fracture on central incisor due to trauma. Eventhough the fracture involves pulp exposure, it remains vital. The tooth lacks sound tooth structure for ferrule effect. The patient wished to retain the tooth. Therefore, initial treatment plan comprised of reestablishing ferrule effect, root canal treatment, endodontic post, and porcelain crown. Nevertheless, due to financial and time constraint, patient refused the suggested treatment plan; thus, direct resin composite bridge was suggested. Case Management: Isolation is followed by calcium hydroxide capping of the exposed pulp. After application of etch and adhesive bonding to tooth 11, 21, and 22, nanohybrid resin composite was layered to fabricate the direct composite bridge, bonded to 11 and 21, with the following layering sequence: 1) palatal; 2) proximal; 3) body; 4) labial. The procedure took 1 hour to restore tooth morphology, function, and esthetics. Upon five years follow-up, the tooth remains asymptomatic. The direct resin composite bridge has been serving well. Conclusion: Direct resin composite bridges can be an affordable, quick, and minimal invasive treatment modality resulting in satisfactory esthetics, function, and longevity.

Abstract

Background: It is not uncommon for a dentist to encounter endodontic cases of severely curved root canals. Performing endodontic treatment on severely curved root canals can be a daunting task due to the notable risk of creating an iatrogenic error including ledge, perforation, or even instrument separation. Therefore, radiograph image analysis, proper endodontic treatment planning, selection of appropriate instruments and technique is essential to ensure success in endodontic treatment of severely curved root canals. Purpose: This case series aims to elaborate the instruments and technique to safely negotiate, clean, shape, and seal root canals with severe curvature. Cases: Three patients came to Universitas Airlangga Dental Hospital due to a severe toothache. After anamnesis and examination, diagnosis was established, and endodontic treatment was deemed necessary. During radiographic image analysis, it is visible that the root canal had a severe curvature. Case Managements: Access opening was performed to allow a straight-line access, orifices were preflared, root canals were negotiated, and glide path was established. Then, shaping, cleaning, and sealing can be performed conveniently until the root canal terminus. Subsequently, coronal restoration was fabricated to restore function and esthetics. Conclusion: With proper knowledge, case analysis, appropriate instruments and techniques, severely curved root canals can be safely and predictably negotiated, cleaned, shaped, and sealed.
