Abstract

Objective : To report a late diagnosed case of posterior uveitis tuberculosis and to create awareness to ophthalmologists for treating ocular tuberculosis as soon as possible. Case Presentation : A 17-year old woman with a complaint of decreased vision in both eyes for the past 5 days. She had a history of non-purulent cough. In the initial examination, the patient was diagnosed and treated with papil edema and chorioretinitis with sub-retinal abscess and swollen disc, but she did not show any improvement. The patient was referred to the other ophthalmologist and diagnosed with ocular and systemic tuberculosis infection with choroidal neovascularization. The infection in the macula affected and aggravated visual acuity of counting finger at 1 meters. Anti-tuberculosis, oral corticosteroid and intravitreal anti-VEGF therapy were performed. The treatment showed significant improvement toward visual outcome and retinal condition. Conclusion : Treatment with oral corticosteroid treatment along the systemic tuberculosis medication and intravitreal anti-VEGF showed good outcome on the right eye but deteriorated on the fellow eye.

Abstract

Introduction: Band keratopathy is usually associated with chronic ocular inflammatory conditions. Recent use of combination treatments such as chelation,excimer laser,and amniotic membrane transplantation in band keratopathy management. Bullous keratopathy (BK) is a main complication of cataract surgery.The purpose of treatment are to reduce pain and improve vision when possible. Treatment depending on the severity of symptoms,cause of BK and potential for visual improvement. BK is a leading indication for keratoplasty and improvement of vision is possible only with keratoplasty. Objective: To report a case of a 64-year-old man with penetrating keratoplasty (PK) following superficial keratectomy (SK), amniotic membrane patch (AMP) and bandage soft contact lenses (BSCL) in band and pseudophakic bullous keratopathy. Case presentation: A 64-year-old man with band and pseudophakic bullous keratopathyreported with reduced vision in both the eyes (1/300 and 6/48 BCVA in the right and left eye, respectively) for past few years.SK, AMP and BSCLwas performed for ocular surface reconstruction in his right eye. One month later, he underwent a PK and3 months following surgery, the corneal graft remained transparent. Six months after the surgery, BCVA of the right eye was 6/30 with S - 3,00 refractive correction. Conclusion: Patients with band and pseudophakic bullous keratopathy can achieve visual outcomes and realise a significant improvement in corneal transparency by undergoing SK, AMP, BSCL and PK.

Abstract

The Ocular Surface Disease Index (OSDI) questionnaire and Dry Eye Questionnare-5 (DEQ-5) are widely used to evaluate subjective symptoms of dry eye disease (DED) as a primary diagnostic criterion. This study aims to compare the validity, reliability, and accuracy of the Indonesian version of OSDI and DEQ-5 questionnaires. Hospital-based cross-sectional study. A total of 100 participants were enrolled in the study. The Indonesian version of the OSDI and DEQ-5 questionnaires were analyzed to evaluate the validity and reliability of both questionnaires. Then, the accuracy of both questionnaires was compared with the diagnostic test and the receiver operating characteristic curve (AUC) analysis. All the question items on both questionnaires are valid and reliable. The validity test using Pearson’s product-moment correlation coefficient, the value exceeds 0.23 (P<0.01) and the reliability test using Cronbach’s alpha coefficient test showed good and excellent internal consistency for OSDI and DEQ-5 (Cronbach’s alpha of OSDI 0.78 and DEQ-5 0.81). The accuracy of both questionnaires was good and there was no significant difference with AUC OSDI=0.78 and AUC DEQ-5=0.79. The Indonesian version of OSDI and DEQ-5 questionnaires met the validity and reliability requirements so that they were declared valid and reliable as effective instruments for DED assessment in the Indonesian population.

Abstract

Dry eye is one of the symptoms that can be suffered by anophthalmic socket eye, which is related to changes in the conjunctival epithelium and inflammatory processes in the anophthalmic socket eye. This study aims to determine differences in the degree of conjunctival impression cytology, tear pro-inflammatory cytokine interleukin-6 (IL-6), and degree of a dry eye between the anophthalmic socket eye and the contralateral healthy eye. This research is a cross-sectional study. Seventeen patients with anophthalmic socket eyes underwent conjunctival impression cytology examination, assessed tear levels of the pro-inflammatory cytokine IL-6, Schirmer test examination, and filled in the Dry Eye Questioner-5 (DEQ-5). The examination was performed simultaneously on the anophthalmic socket eye and the contralateral healthy eye. This study showed a significant difference in conjunctival impression cytokine (p= 0.004) and a significant difference in tear pro-inflammatory cytokine IL-6 levels (p= 0.014). There was no significant difference (p=0.319) in the results of the Schirmer 1 test, but there was a significant difference in the Schirmer 2 test (p=0.0047). There is a significant difference in the value of the DEQ-5 questionnaire (p = 0.003). There were significant differences in conjunctival impression cytology, tear pro-inflammatory cytokine IL-6, and degree of the dry eye between the anophthalmic socket eye and the contralateral healthy eye. Therefore, it is necessary to administer artificial tear drops to each patient with anophthalmic socket as a preventive measure for dry eye in the anophthalmic socket.
