Abstract

Objective: to evaluate walking performance in female with type 2 diabetes mellitus (DM2) with the 400-meters walk test (400-MWT) compared to healthy individuals.Methods: two groups of female subjects with DM2 and healthy individuals were matched by the age. The 400-MWT parameters to be compared were walking speed (WS) and predicted maximum oxygenconsumption (pVO2max). Baseline examination included body mass index (BMI), random blood glucose (RBG), and ankle-brachial index (ABI). All subjects performed 2 minutes warm up before thetest. Heart rate (HR) was recorded every 30 seconds, and blood pressure (BP) was measured before warm up and within 60 seconds after test. The test was performed twice on a different day.Results: Nineteen subjects on each group participated in the study. The mean WS was significantly different (p<0.0001) between the study group (1.26 + 0.19 and 1.31 + 0.17 m/s) and the control group(1.70 + 0.20 and 1.78 + 0.24 m/s) for the first and second tests respectively. There was a significant difference of mean pVO2max (p<0.0001) between the study group (17.22 + 2.94 and 17.99 + 2.36 ml/kg/min) and the control group (23.68 + 3.79 and 24.44 + 3.74 ml/kg/min).Conclusion: the 400-MWT demonstrated lower walking performance in female subjects with DM2 compared to healthy individuals.Keywords: Type 2 DM, healthy individuals, 400 meter walk test, walking speed, VO2max.

Abstract

Background: Hypertension  is one  of  the  degenerative  diseases which  killed  about 7,2 mill people,  about 13 % of the total death worldly.  In Indonesia  the disease has become more popular, It was  reported  that Indonesia had got a quite high prevalence of hypertension , among  male 24,9%, female 23,6% , elderly 33,3 % , SUTETI 500 kV is  the  Extra High  Electrical Energy  Transmission  lines    transferring    the   energy  from  the Electrical Power Installation   to the consumers via  the connection of Power   Relay station.  The  SUTETI electromagnetic exposure many  times  has  been  connected with  diseases  and mental  emotional  disorders  used  to  be experienced  by patients with hypertension  such as  head ache,  dizziness, easily  get angry,  heavy neck  as well  as stroke. These health disturbances  have been  considered by many  people  as SUTETI’s  negative health  impact among  people living underneath.   Most of the times were used by public as the major reasons against the installation of SUTETI wire  network.Method: The aim of study  is  to  identify  the  relation between SUTETI exposure and hypertension, and  its  related factors  among population  living underneath. Another objective  of study  is  to  identify  the  relation between mental emotional  factors and hypertension. The population was people living underneath SUTETI 0-100 ms  right and  left away  from  the  towers. The number  of  samples  collected was  about 2000  people with  the calculated  sample size 753 and were stratified  into  three zones, zone 1  , 0-30 ms  right and  left  from  the  tower, zone 2  , 30-70 ms and  zone 3  ,70-100 ms.  The  independent  variables   were sociodemographic  factors  i.e.,  age,  sex,  education, income  ,  length of stay at home, duration of stay at  the  location, besides smoking habit, annoyance, satisfaction, electrical  field dose  , magnetic  field dose  and zone  .   Blood chemistry examined were  cholesterol and nutritional status  besides melatonin, mental emotional  and  sleeping  disorders.Result  and  conclusions: Study  results  reported  that  the  highest  outside  the  house measurements  of  the electric  field was  in zone 1  ,   3,2 kV/m   and magnetic  field   was     6,7 X 10-² mT  , both were still  lower  than  the National Standard of SUTETI in  Indonesia  – SNI 2003 , 5 kV/m and 0,1 mT. The prevalence of hypertension based on  zone were  significant  ,  zone 1,  14,14%, zone  2  ,   11,31%  and zone  3,  5,66 %  (   p  =  0,004)   The  logistic regression  function analysis  identified  two determinants, duration of  stay  ( p =  0,044  )  ; OR 3,85  (95 %CI  :1,04 – 14, 12) ; income ( p = 0,019 , OR  15, 891 ( 95% CI: 1,578 – 169,97). People living > 30 years under the SUTETI towers  (0  – 100ms  ) will  be  risky  3,85  times  to get  hypertension  than  people  living  less  than  30  years. Also people with less income had the risk 16 times to get hypertension compared to middle income. The results did not reveal  any  specific  evidence  of  association  between  the  electromagnetic  exposure  and  hypertension.  The logistic  regression  function  also  did  not  identify  any  association  between mental  emotional  disorders  and sleeping  disorders  towards  hypertension.Keywords:    hypertension, SUTETI, mental emotional  disorders

Abstract

Background and Method: A study on the health impact of the Extra High Voltage Transmission power line, 500 kV (SUTETI) was carried out among 1750 people who have been living for more than 15 years around the tower in Jakarta and Tambun. The dependent variables were health impact based on identification of laboratory exams on blood chemistry, electrocardiogram, adult roentgen photo, bone photo roentgen of children, hypertension as well as mental emotional disorders using Cornell Medical Index (CMI) and Sleeping Disorders. The melatonin enzyme was  measured trying to identify the association with electromagnetic field exposure and mental psychosomatics evidence.  The independent or risk factors measured besides the socio demographic, duration of stay, length of stay at home, smoking were the respondents' annoyance and satisfaction living around the tower and the reasons of feeling not safe. At the same time the electromagnetic field was also measured separately. The personal electric and magnetic field doses were the function of length of stay inside the house and the electric field and magnetic field measured inside the house. The study location was divided into three zones, zone 1, 0-30 ms left and right sides from center of tower foot, zone 2, 30-70 ms and zone 3, 70-100 ms.Results and Conclusion: Study results reported the range  of  electric field measured , at one meter from the ground, showed the highest 3,2 kV/m just underneath the tower (10-30 m), declined to  0,5 kV/m  about 70 m away from the center of the tower and steadily declined, about 70-100 m, the electric field became very small to 0,0 kV/m The magnetic field measured were, the highest 20-30 m, 534 mA/m ( 6,7x 10 -² mT),  declined at the distance of 70 m to 110 mA/m ( 1,3 x  10 -² mT ), very small  at a 100 m, 50 mA/m ( 6,3 x 10 -3 mT). Study results also revealed that there were no significant associations identified  between  adults roentgen results, children bone roentgen results, hypertension, mental emotional disorders/ CMI, sleeping disorders with electric and magnetic field dose . The logistic regression functions reported the magnetic field dose as a borderline determinant to electrocardiogram. People who lived 0-70 m had 7 times greater risk to suffer from emotional mental disorders than those living >70 m away from the tower even though mental emotional disorders was neither significant to electro field dose nor to magnetic field dose. The melatonin correlates positively with electromagnetic doses which mean melatonin was not suppressed by the electromagnetic exposures.
