Abstract

Hipertensi merupakan peningkatan tekanan darah secara kronik. Pembiayaan kesehatan di Indonesia semakin meningkat, maka perlu analisis efektivitas biaya untuk memutuskan pemilihan obat yang efektif secara manfaat dan biaya. Penelitian ini bertujuan untuk mengetahui antihipertensi yang paling cost-effective di RSUD Kota Madiun. Penelitian dilakukan secara retrospektif terhadap rekam medik pasien. Aspek pengambilan data meliputi data terapi dan biaya pengobatan. Data yang diperoleh diolah dengan analisis descriptive. Efektivitas pengobatan diukur berdasarkan penurunan tekanan darah sistolik dan diastolik yang mencapai target, sedangkan efektivitas biaya dilihat berdasarkan nilai ACER dan ICER.Hasil penelitian menunjukkan persentase efektivitas terapi obat A (ACEI dan CCB) sebesar 98% sedangkan obat B (ARB dan CCB) sebesar 90%. Rata-rata total biaya antihipertensi obat A (ACEI dan CCB) sebesar Rp.3.738.972,22 dan obat B (ARB dan CCB) sebesar Rp.4.170.392,72. Kelompok obat A (ACEI dan CCB) lebih cost-effective dengan nilai ACER sebesar Rp.38.152,77 dibandingkan obat B (ARB dan CCB) sebesar Rp.46.337,68 pada pasien hipertensi rawat inap di RSUD Kota Madiun. Kesimpulan dari penelitian ini yaitu kelompok terapi A (ACEI dan CCB) memiliki efktivitas terapi yang tinggi dengan biaya lebih rendah. Harapannya rumah sakit dapat mempertahankan anggaran obat untuk penyakit hipertensi secara efektif dan efisien. Kata Kunci : Hipertensi, Efektivitas Biaya, Antihipertensi Kombinasi

Abstract

Dyslipidemia is one of the main risk factors for cardiovascular disease, and cholesterol management therapy has significant benefits in the primary and secondary prevention of coronary heart disease and stroke. Therefore, lipid-lowering therapy, especially using HMG-CoA reductase inhibitors (statins), aims to assess the effectiveness of using statin-class dyslipidemia medications, such as simvastatin and atorvastatin, in reducing LDL levels in cardiovascular patients at X Hospital. This research method uses an observational analytic study with a retrospective research design. This study employs the purposive sampling method. The results of this study show that the majority of patients are female (75%). Based on age grouping, it was found that the highest proportion of patients were aged 56-65 years, accounting for 45%. The use of dyslipidemia drugs that achieved the target LDL reduction ≥ 50% includes Simvastatin 10 mg (33.82%), Simvastatin 20 mg (11.94%), Atorvastatin 10 mg (10.45%), and Atorvastatin 20 mg (41.79%). The most effective drug in achieving the LDL reduction target was Atorvastatin 20 mg.

Abstract

Inflammation is a protective response that is intended to eliminate the initial cause cell injury and cell discard and necrotic tissue caused by cell damage. Plants suspected of having anti-inflammatory properties as is Herba Celery (Apium graveolens L.) which included family Apiaceae and leaves Binahong (Anredera cordifolia (Ten.) Steenis) are included in the family basellaceae. Which act as antiinflammatory herbs binahong celery and leaves are flavonoids. This study aims to determine the effect of anti-inflammatory herb extract combination Celery (Apium graveolens L.) and leaves Binahong (Anredera cordifolia (Ten.) Steenis) against a white male rats. Herbal extract and leaf celery binahong extracted using 96% ethanol by maceration method. Animal test used was a white male rats as much as 25 animals with body weight of 100-300 grams. The test animals were divided into 5 groups: negative control group (CMC 1%), the positive control group (Diclofenac Sodium 50 mg), group combination herbal extracts celery and leaves binahong with a dose of 200 mg / kg: 200 mg / kg, 100 mg / kg: 300 mg / kg, 300 mg, kg: 100 mg / kg. Treatment 1 hour before the feet of mice induced by subplantar karagenin on the rear left foot white male rats. Edema volume measurements on the soles of mice were observed every 1 hour for 5 hours. The results showed a combination of herbal extracts and leaf celery binahong show stable results, where the hour-3 increased volumes continued in the hours 4th and 5th. Results per cent udema on a combination of herbal extracts and leaf celery binahong ie 49.13%, 44.66% and 45.92%. The average volume is used to calculate the percent edema inflammation, inflammation percent results obtained to calculate the percent inhibition.
