Abstract

The use of herbal medicine in the formal health care requires adequate scientific evidence especially on herbal safety data. The herb could be used for the treatment of anemia. The indicator for the safety of the herb was kidney function by measuring urea and creatinine. A clinical trial study with pre-post test designed was conducted at the Jamu Research Center "Hortus Medicus" Tawangmangu to assess renal function of patients treated anemic with herbal formula for anemia. A total of 35 subjects volunteered to follow this study received therapy with herbal formula for 1 month. Formula was a herbal infusion made up of 5 grams of red amaranth leaves (Amaranthus tricolor L.), 10 grams herbs of tapak liman (Elephantopus scaber L.) and 15 grams of temulawak rhizome (Curcuma xanthorrhiza), taken 3 times a day. Kidney function was observed by measuring the levels of urea and creatinine subjects at baseline (D-0) and at day 28 (D-28). The results of t-test between urea levels in D-0 and D-28, showed the value of p = 0.942 (> 0.05) and creatinine levels in D-0 and D-28, p-value = 0.935 (> 0.05), which means no significant difference. It can be concluded that herbal formula for anemia did not affect renal function.

Abstract

Research on the safety of Jamu (herbal medicine) formula is important to guarantee its use in community. The Jamu Saintification development program has been resulted a cholesterol-lowering herb containing of 7 plants. This study aimed to determine the safety of the cholesterol-lowering herb. This study is a phase one of clinical trial with a pre-post group design, a single arm, involving 50 subjects with mild hypercholesterolemia without any complications and comorbidities. The intervention was conducted for 28 days at the Hortus Medicus Herbal Medicine Research House (RRJ) in 2014. Outcome parameters included clinical signs and symptoms, liver function laboratory tests (SGOT, SGPT), renal function (ureum, creatinine) and routine blood counts (hemoglobin, leukocyte, platelets, hematocrit). Subjects were requested to boil the herb everyday and drank it twice a day. The data were analyzed descriptively and statistically using SPSS. The results showed increasing in bowel frequencies of 92% subjects and abdominal pain in bowel movements of 34% subjects. These effects were still in a normal range and can be tolerated by the subjects. The paired t test analysis showed no differences in average laboratory parameters on day (D)-14 and D-28 compared to the baseline (p > 0.05). The cholesterol-lowering herbal medicine does not change the function of the liver, kidneys and blood, with mild side effects of increasing the frequencies of bowel movements and stomachaches.

Abstract

Salah satu penyakit tidak menular yang memiliki prevalensi tinggi di dunia adalah hipertensi. Selain menggunakan pengobatan konvensional, penatalaksanaan hipertensi dapat ditangani dengan jamu. Penelitian bertujuan memeroleh gambaran penggunaan, persentase keberhasilan terapi dan jenis kelamin serta frekuensi terapi, dengan keberhasilan terapi ramuan jamu antihipertensi di Rumah Riset Jamu (RRJ) Hortus Medicus. Desain penelitian menggunakan potong lintang dengan rancangan observasional deskriptif. Observasi dilakukan pada data sekunder berupa rekam medik. Ramuan antihipertensi yang digunakan di RRJ periode Januari-Juni 2017 adalah ramuan I terdiri dari tanaman obat Apium graveolens‚ Centella asiatica‚ Orthosiphon stamineus dan Imperata cylindrica dengan 70 resep (70%). Ramuan II terdiri dari Apium graveolens‚ Centella asiatica‚ Orthosiphon stamineus‚ dan Imperata cylindrica dan Sonchus arvensis dengan 20 resep (20%). Ramuan III terdiri dari Apium graveolens‚ Centella asiatica dan Orthosiphon stamineus dengan 10 resep (10%). Persentase keberhasilan terapi ramuan jamu 1‚ 2‚ dan 3 berturut-turut adalah 80%‚ 60%‚ dan 50%. Faktor-faktor yang mempengaruhi keberhasilan terapi adalah jenis kelamin dan frekuensi terapi. Jenis kelamin tidak memiliki korelasi yang signifikan dengan keberhasilan terapi hipertensi dengan nilai p=0‚705 (p>0‚05)‚ dengan nilai Odds Ratio (OR) sebesar 1‚151. Faktor frekuensi terapi juga tidak memiliki korelasi yang signifikan dengan keberhasilan terapi hipertensi dengan nilai p=0‚420 (p>0‚05)‚ dengan nilai Odds Ratio (OR) sebesar 3‚421.

Abstract

Background: Osteoarthritis (OA) of the knee joint has the highest prevalence among all types of rheumatic diseases. The most common symptom of knee OA is pain around the joint. Patients with OA require long treatment, especially using analgesic and anti-inflammatory drugs. Turmeric is efficacious as a pain reliever and anti-inflammatory. This study aims to estimate the magnitude of the effect of turmeric extract (Curcuma domestica) on pain in patients with knee OA compared to placebo and non-steroidal anti-inflammatory drugs (NSAIDs), based on the results of a number of previous similar studies.Subjects and Method: This study is a systematic review and meta-analysis with the following PICO, population: patients with knee OA. Intervention: turmeric extract. Comparison: placebo and NSAIDs. Outcome: pain reduction. The articles used in this study were obtained from three databases, namely Google Scholar, Pubmed, and Science Direct. Keywords to search for articles: “Knee Osteoarthritis” OR “Knee OA” AND “Curcuma longa” OR “Curcuma domestica” OR “Turmeric extract” AND “Placebo” OR “NSAID” AND “pain” OR “visual analogue scale” OR “ VASE". The articles included are full-text with RCT study designs from 2011 to 2021. The selection of articles is carried out using PRISMA flow diagrams. Articles were analyzed using the Review Manager 5.3 application.Results: A total of 15 RCT studies were selected for systematic review and meta-analysis. Data collected from 10 studies showed that patients with knee OA who received turmeric extract experienced 1.60 units lower joint pain than placebo (SMD= -1.60; 95% CI= -2.23 to -0.97; p<0.001). Data from 5 studies showed that patients with knee OA who received turmeric extract experienced 0.06 units higher joint pain than NSAIDs. The comparison between the turmeric extract group and the NSAID group did not show any significant difference (SMD= 0.06; 95% CI= -0.10 to -0.22; p= 0.490).Conclusion: Administration of turmeric extract was more effective than placebo and not inferior to non-steroidal anti-inflammatory drugs in reducing pain in patients with knee OA.Keywords: osteoarthritis, turmeric, painCorrespondence: Danang Ardiyanto.
