Abstract

Background: The incidence of skin cancer has been increasing over the past decades globally. At present, there is no valid national cancer data in Indonesia. The purpose of this study is to evaluate the increment of skin cancer cases at Dr. Cipto Mangunkusumo National General Hospital, Jakarta. Methods: In this retrospective descriptive study, we evaluated all pigmented skin tumor cases from June 2014 to June 2017. Data were obtained from the Department of Anatomical Pathology and medical records at Dr. Cipto Mangunkusumo National General Hospital. Data were compared with previous reports. Results: From 263 skin cancer cases, the most frequent skin cancer was basal cell carcinoma (BCC; 66.9%), followed by squamous cell carcinoma (SCC; 27.4%), and malignant melanoma (MM; 5.7%). Most of the skin cancer were predominantly in the female population. In BCC and SCC, most of the patients were within the age group above 60 years (n = 106 and 18, respectively). In MM, the incidence is greater in age group of 41–50 years (n = 5). For BCC and SCC, the distribution sites of lesions were mostly in sun-exposed areas, whereas the distribution sites of MM were mostly in non-sun-exposed areas. Median diameters of BCC, SCC, and MM were 2, 4, and 6.5 cm, respectively. There was an increase of BCC incidence from 1996 to 2017. Conclusion: In comparison with other types, the incidence of BCC in Dr. Cipto Mangunkusumo National General Hospital is increasing. A well-documented skin cancer registry is required to establish national data of skin cancer in Indonesia.

Abstract

Introduction: Despite its rarity, nail involvement in Langerhans cell histiocytosis (LCH) may show various clinical presentations. This study aims to show the roles of nail involvement in LCH patients as the diagnostic and prognostic clues.Case illustrations: We presented four cases of multisystem LCH in children which were already confirmed by skin biopsy with various nail abnormalities. We were able to perform nail biopsy in two patients and confirmed the nail involvement. Histopathological examination showed the infiltration of Langerhans cells characterized by indented/reniform nuclei and CD1a expressions. All patients had high-risk organ involvements. Discussion: Langerhans cells may infiltrate the nail bed, proximal nail fold, and nail matrix. Further infiltration may destruct the nail plate. Hypothetically, we suggest that the nail bed as the initial infiltration site of Langerhans cells. The different sites of involvement lead to different clinical presentation. Nail abnormalities may predict a poorer prognosis, as they mostly occur in patients with multisystem disease. Conclusions: Nails should be routinely inspected in the suspicion of LCH. The presence of nail abnormalities in LCH patients may predict a poorer prognosis.

Abstract

Background Cutaneous melanocytic lesions have variety of subtypes ranging from benign (nevus) to malignant lesions (malignant melanoma). Differentiating benign and malignant melanocytic lesions is challening, particularly due to overlapping characteristics. This study was conducted to determine clinicopathological characteristics of cutaneous melanocytic lesions and the prevalence ratio of each characteristic for predicting malignant behavior. Method Cross-sectional study was conducted with data retrieved from Anatomic Pathology Department of Faculty of Medicine Universitas Indonesia and Dr. Cipto Mangunkusumo Hospital. Data of 145 histopathology-confirmed cutaneous melanocytic lesions cases between 2018 – 2022 were sampled and collected. Bivariate analysis was used to determine the prevalence ratio of each characteristic to predict malignant behavior. Result Out of 145 cases, 14.5% were malignant lesions (melanoma) and the rest were benign (nevus). Clinicopathology features and characteristics were correlated with malignant behavior, such as cytological atypia (p <0.01), dermal mitosis (PR=91.5; p<0.01), asymmetry (PR=31.8; p <0.01), suprabasal melanocyte (PR=21.4; p <0.01), lower extremity location (PR=6.5; p <0.01), sized >6 mm (PR=5.2; p <0.01), and male gender (PR=3.3; p <0.01). Conclusion Clinicopathological characteristics of cutaneous melanocytic lesions such as cytological atypia, dermal mitosis, asymmetry, suprabasal melanocyte, lower extremity location, sized >6 mm, dermal lymphocytic infiltrate, and male gender were predictive to predict malignant lesions.
