Abstract

Background: Burn injuries are associated with high mortality and morbidity. The incidence of death resulting from burns can be influenced by various prognostic factors, with nutritional status being a significant contributor. Therefore, assessing nutritional status through the calculation of body mass index (BMI) and post-resuscitation albumin levels becomes imperative. Purpose: This study aims to explore the correlation between BMI and post-resuscitation albumin levels with the mortality of burn patients at Prof Ngoerah General Hospital during the period 2020-2022. Methods: This research adopts an observational analytical approach, utilizing a cross-sectional research design and consecutive sampling techniques. The study relies on secondary data, specifically medical records of burn patients. Out of a total of 134 medical records, 100 were selected based on adherence to inclusion and exclusion criteria. Results: The study reveals median values for age (42.5), BMI of 23.9 and albumin levels 24 hours post resuscitation recorded at 2.235. The Spearman test analysis conducted on 100 samples reveals a notable correlation between these variables and the mortality of burn patients (r = 0.263; PR 1.707; 95% CI 1.163 – 2.504; p = 0.008). The chi-square test analysis regarding the association between post-resuscitation albumin levels and burn patient mortality also demonstrates a significant relationship (PR 2.390; 95% CI 1.358 – 4.207; p = 0.000). Conclusion: In conclusion, there is a statistically significant association between mortality and both BMI and post-resuscitation albumin levels among burn patients treated at the Prof Ngoerah General Hospital during the period 2020-2022. Abstract

Introduction : The technique for repairing a cleft palate depends on the length and width of the palate and the type of cleft palate according to the Veau classification. The most common surgical techniques include the Furlow double opposing Z-plasty, the Bardach palatoplasty, the von Langenbeck palatoplasty, and the V-Y pushback. One of the most frequent complications after cleft palate repair is an oronasal fistula (ONF). This article aimed to compare the risk of postoperative ONF formation following cleft palate repair. Method : We searched for manuscripts involving patients with ONF formation following the Furlow, Bardach, von Langenbeck, and V-Y pushback palatoplasty. Electronic literature searching of the PubMed, Scopus, and Cochrane databases was conducted for the studies published up to November 2022. Result: The meta-analysis used 17 studies, including the analysis of 3,207 repaired cleft palates. The Furlow technique was associated with a statistically lower risk of ONF formation when compared to the von Langenbeck and V-Y pushback techniques (RR=0.06[0.43-0.840],p<0.01 and RR=0.30[0.15-0.62],p<0.01 respectively). There were no statistically significant differences regarding ONF formation between the Furlow technique and the Bardach technique (RR=1.45[0.48-0.43],p=0.51). Conclusion: The Furlow technique is associated with a lower risk of ONF compared to the von Langenbeck and V-Y pushback techniques. There were no statistically significant differences in ONF formation between the Furlow and Bardach techniques.

Abstract

Introduction : Nasal reconstruction presents a challenge in case of patients with unilateral cleft lip and palate (UCLP). Correction of the nasal deformity to achieve a symmetrical nose from the deficient columella and malformed nasal cartilage in cleft patients presents a great challenge. Nasoalveolar molding (NAM) was developed as a neoadjuvant therapy for patients with cleft lip and palate to improve nasal deformity before surgical intervention. The aim of this systematic review was to understand more about the effect of NAM on nasal symmetry and proportions in patients with UCLP. Method : We searched for manuscripts involving patients with UCLP who received NAM prior to cleft lip repair. Electronic literature searching of the PubMed, Scopus, Cochrane and Google scholar databases was conducted for the studies that had been published up to December 2021. Result : Presurgical NAM demonstrated benefits in nasolabial form when compared with patients who did not receive any presurgical infant orthopaedic appliances (PSIO). The use of presurgical NAM helps to achieve favourable reshaping of the nose and decrease the severity of the initial cleft deformity, resulting in improvement on nasolabial form. These changes lead to improved nasal symmetry and proportions in UCLP patients. Conclusion: The preponderance of evidence in this review suggests that presurgical NAM is a beneficial adjuvant therapy for nasal deformity correction and helps achieve nasal symmetry in patients with UCLP. However, there is insufficient evidence to conclude whether presurgical NAM produces these benefits at the time of nasal maturity.

Abstract

Background : Anthropometry is a non-invasive, inexpensive, and objective method for evaluating orofacial morphology. This method has clinical applications in myofunctional assessment and therapy. Photogrammetry is a reliable and accurate method for measuring certain facial dimensions. In Indonesia, to date there has been no research that has conducted a photogrammetric evaluation of lip anthropormetry. Method : This study was conducted at the Faculty of Medicine, Udayana University which began in May to October 2024 with research subjects, namely 90 first-year undergraduate medical students, Faculty of Medicine, Udayana University. Data in the form of anthropometry, race, history of congenital abnormalities, facial trauma, siliconoma, age, gender and smoking were recorded and then analyzed. Result : The results of the analysis of lip anthropometry based on gender showed a significant difference in lower lip height, where in women the average was 3.25 (0.36) and in men 3.01 (0.30), with a p value of 0.001. Likewise, the lower lip height was found to be significantly different with a p value = 0.003. Upper vermilion height in women was 1.51 (0.35) while in men it was 1.36 (0.25), these results were statistically significant (p value = 0.041). However, the comparison of lower vermillion did not show any significant difference between the two groups. Cutaneous lower lip and philtrum width were also significantly different with p values of 0.004 and 0.015, respectively. Conclusion: The results of this study are expected to provide an overview of the analysis of anthropometric measurements of the lips using the photogrammetry method as basic data for measuring ideal lip anatomy in the field of facial reconstruction in Indonesia. Abstract

The consequences of post-burn neck contractures are considered the greatest challenge in reconstructive surgery. This deformity affects the neck area thereby limiting head movement. These complications can be associated with loss of articulation, cosmetic disfigurement and psychological changes that impair quality of life and result in loss of productivity. Therefore, an appropriate surgical management are essential to obtain optimal results. ALT (anterolateral) free flap can be used to resurface the defect, this technique provides a long pedicle with adequate lumen diameter of blood vessels. The z-plasty technique is useful for redirect a scar into better alignment with a natural skin fold or the lines of least skin tension. Split-thickness skin graft (STSG) is a preferred technique for large wound areas and relatively avascular sites. This study aims to report our experience in reconstructing burnt mentosternal contractures with ALT free flap, z-plasty and STSG. We present a clinical case of a thirty-five years old male with post-burn mentosternal contracture ONAH classification type III, dermogenic type diffuse flexion contracture of the colli region, anterior thorax region, and left and right superior extremity regions, dermogenic type linear contracture of the left and right anterior axillary regions, and burned ear deformity in the left and right auricular due to a history of burns from twenty-four years ago. A contracture release procedure was performed with ALT free flap reconstruction, z-plasty and STSG. After the reconstruction, a pleasing outcome was accomplished in terms of both functionality and esthetics. A severe mentosternal contracture was released, and the limited movement of the neck and both extremities was greatly lessened. Mentosternal contracture reconstruction with ALT free flap, z-plasty and STSG showed a good outcome in this case.
