Abstract

In Japan, no support system for mothers with developmental disabilities is currently in place. This study aimed to clarify the actual situation of support for pregnant and puerperal women with developmental disabilities in obstetrics and gynecology-related facilities. A self-administered questionnaire survey including free-form questions was distributed to 120 obstetrics and gynecology facilities in Prefecture A. The questionnaire responses were analyzed using descriptive statistics. The free descriptions were categorized and analyzed inductively. The valid response rate was 40.8% (n = 49). Twenty-five facilities reported dealing with pregnant and puerperal women with developmental disabilities, more than half of which provided support. None of the facilities had guidelines or pamphlets. Medical professionals felt a [lack of knowledge] and [difficulty in dealing with] support for mothers with developmental disabilities, and demanded the [construction of a support system].The present findings revealed that all facilities had problems with support systems and regional cooperation. Therefore, the establishment of a support method suitable for pregnant and puerperal women with developmental disabilities is needed.

Abstract

日本の助産師のケアは、すべての母親の乳房問題を解決することはできません。この定性的、フォーカスグループインタビュー研究は、乳房ケアルーチンへの乳房超音波の導入に関する助産師の意見を明確にすることを目的としていました参加者は、5人と6人の助産師の2つのグループに分かれました。データは、フォーカスグループインタビューを実施することによって収集されました。インタビューデータは定性的に分析された。乳房ケアのための超音波の導入の3つの理由が助産師に明らかにされた。1つ目は、一定の助産能力の必要量を失い、母親の負担が増大する可能性であり、現状に不利な状況であった。2つ目は、助産師のスキルとケアの効率を向上させることによって母親の心理的安定性を高める可能性という肯定的なものでした。3つ目は、乳房超音波装置が簡単に操作できるように改良し、スキルを向上させるためのトレーニングを提供することです。助産師には、乳房ケアに乳房超音波を導入する利点が示されている。乳房超音波を乳房ケアに導入するためには、助産師が乳房超音波の操作方法を学ぶためのトレーニングコースを開発する必要があります。

Abstract

Objective: This study aims to reveal the types of support that midwives provide to mothers during the postpartum period after infertility treatment.Methods: Sixteen midwives, who had experience dealing with mothers in the puerperium after infertility treatment, participated in semi-structured interviews.Results: To determine the roles of mothers, the following nine categories regarding midwives’ support for puerperal women after infertility treatment were identified: 1) midwives listen more carefully to puerperal women who have undergone infertility treatment than to those who have experienced natural childbirth, 2) midwives encourage adding milk when breast milk is insufficient for the baby’s growth, 3) midwives assist women who have undergone an emergency caesarean section overcome the feeling of regret and remain positive, 4) midwives recognize that there are many elderly women, 5) midwives’ level of support during the puerperium after natural pregnancy and after infertility treatment is identical, 6) midwives are involved in the step-by-step process of the mother becoming a parent, 7) midwives are involved in raising the child positively, 8) during the semi-monthly and monthly health check-ups, midwives encourage and instill confidence in mothers, and 9) midwives help mothers cope with anxiety regarding the lack of or poor weight gain among their children.Conclusion: Midwives play a significant role in reducing anxiety among puerperal women that have undergone infertility treatment by providing consistent emotional support to such women, even after discharge.
