Abstract

Latar Belakang : Prevalensi terjadinya ulkus diabetikum di Indonesia sebesar 15% dan sering kali berakhir dengan kecacatan dan kematian (Waspadji, 2006). Penyembuhan luka sangat dipengaruhi oleh metode perawatan luka. Banyak tehnik perawatan luka.Salah satunya dengan menggunakan air garam.Garam merupakan benda padatan berwarna putih berbentuk kristal yang merupakan kumpulan senyawa dengan bagian terbesar Natrium klorida (NaCl)  lebih dari 80 % serta senyawa lainnya seperti Magnesium klorida, Magnesium Sulfat, kalsium klorida dan lain-lain. Garam mempunyai sifat  yang mudah menyerap air.Ketika garam dicampur dengan air, terutama air hangat, terbentuklah partikel-partikel dengan muatan listrik yang berbeda: ion natrium yang bermuatan positif dan ion klor yang bermuatan negatif. Ketika larutan garam ini diberikan pada bagian yang luka, ion-ion ini akan mengatur tekanan sel-sel di sekitar luka. Tekanan diatur sedemikian rupa sehingga cairan tidak akan keluar dari dalam sel. Dengan kata lain, luka akan menjadi cepat kering.Di samping mengeringkan luka, air garam juga dapat membunuh bakteri yang menyerang luka (terutama bakteri staphylococcus dan streptococcusTujuan dan desain penelitian: tujuan penelitian untuk mengetahui pengaruh rendam air garam terhadap proses penyembuhan luka ulkus diabetikum. Desain penelitian ini menggunakan quasi eksperimen dengan rancangan randomized control group only design. Populasi pada penelitian ini adalah seluruh pasien dengan ulkus diabetikum di RS Prima Medika Pemalang. Sampel yang diambil dalam penelitian ini adalah dari populasi. Teknik pengambilan sampel yang dipakai dalam penelitian ini adalah quota sampling. Pada penelitian ini sampel berdasarkan kuota yang telah di tetapkan peneliti yaitu sejumlah 40 responden, 20 responden dilakukan tindakan rendam air garam, dan 20 responden tidak dilakukan rendam air garam sebagai kontrol.Analisa : analisa menggunakan univariat dan bivariate. Analisa bivariat dilakukan dengan menggunakan uji beda mean independent menggunnakan uji non parametric Mann-Whitney U karena distribusi data tidak normal.Hasil : menunjukkan bahwa rata-rata nilai proses penyembuhan luka pada kelompok intervensi (direndam air garam) adalah 3.70, sedangkan rata-rata nilai proses penyembuhan luka pada kelompok kontrol (tidak direndam air garam) adalah 3.35. Hasil uji statistik didapatkan ada pengaruh perawatan luka rendam air garam terhadap proses penyembuhan luka ulkus diabetikum (p value = 0.029) Kata kunci       : Rendam air garam, proses penyumbuhan ulkus diabetikum

Abstract

Background: Puerperal infection is caused by the entry of bacteria through uterine or perineal wounds. These bacteria can trigger proinflammatory cytokines. Excessive proinflammatory response results in tissue damage, septic shock to death. Among the non-pharmacological methods are extracts of red betel leaves which have phytochemical screening of flavonoids and tannins as antibacterial and antiseptic.Purposes: Knowing the potential of simplicia extract of red betel leaf on increasing the effectiveness of antibiotic in postpartum (study experiment of the process of healing of perineal wounds).Method: Quasi-experimental post-test only with control group design. Subjects were postpartum mothers with episiotomy perineal wounds that were sewn at Ummu Hani Purbalingga Mothers and Child Hospital using purposive sampling with a total of 38 respondents. The treatment group was given amoxicillin and red betel leaf simplicia extract, the control group was given amoxicillin and perineum wound care standard service. Provision of intervention carried out for 7 days, the measurement instrument for perineal wound healing is the REEDA score. Data analysis using independent t-test.Result: An independent test between the two groups found a mean difference in perineal wound healing p-value 0.0001. On intervention group the mean of perineal wound healing in day 5 and control group day 7.Conclusion: There is potential for simplicia red betel leaf extract to increase the effectiveness of antibiotic in postpartum.

Abstract

Background: Diabetes is a chronic disease that will be suffered for life and can have an impact on complications, one of which is Peripheral Arterial Disease (PAD) which is characterized by a decrease in the value of the Ankle Brachial Index (ABI) and is the main cause of the increasing number of gangrene incidents and amputations. To prevent these complications, it is necessary to have therapy that can increase the value of the Ankle Brachial Index (ABI), which is by combining acupressure therapy and warm water foot soak (hydrotherapy). Purpose: To analyzing the effectiveness of the combined therapy of acupressure and warm water foot soak towards Ankle Brachial Index (ABI) value in type II diabetes mellitus. Methods: Using a quasi-experimental design with a pretest-posttest design approach with 51 participant. The participant were taken using simple random sampling technique and divided into 3 intervention groups. The first group is acupressure (n = 17), the second group is warm water foot soak (n = 17) and the third group is a combination of acupressure and warm water foot soak (n = 17). The normality test in this study was conducted using the Shapiro Wilk test, the dependent t test was used to determine any changes in each group while comparing the effectiveness of Ankle Branchial Index (ABI) values of each group using the one way anova test. Results: The use of acupressure therapy can have an effect on the value of the Ankle Brachial Index (ABI) with a significance value of p:  .000. The group receiving warm water foot soak therapy also experienced changes but the change in the Ankle Brachial Index (ABI) value was not too significant because the significance value was obtained p:  0.002. While in the group with the combination of acupressure and warm water foot bath has a significance value of p: 0.000 Conclusion: The combination therapy of acupressure and warm water foot soak provides an effective influence on the ankle branchial index with a significance value of p: 0.000 (p < 0.05) compared to warm water foot soak therapy which has a significance value of p: 0.002. (p< 0,05). By combining acupressure therapy and warm water foot soak, sclerosed blood vessels can stretch due to the effect of heat which causes vasodilation of blood vessels which has an impact on smooth blood flow and can carry oxygen and nutrients well, especially to the feet.

Abstract

Background: Hyperglycemia often occurs in DM patients even though they have received pharmacological therapy, thus encouraging people to consume herbal medicines. The dosage of herbal therapy consumed by the community is not appropriate, only the dosage of community assumptions, resulting in negative impacts. Chronic hyperglycemia causes worsening of the Ankle Brachial Pressure Index (ABPI) score. Giving red betel extract with the right dose can be a solution, because red betel extract contains flavonoids that can inhibit y-amylase and y-glucosidase so that the body can control blood sugar levels and improve ABPI scores. Purpose: To determine effectiveness of red betel extract on fasting blood sugar levels and ABPI. Method: A quasy experiment approach, specially focusing on pretest and posttest group. The study took place from October-November 2024, in Padangsari and Srondol Health Centers, Semarang. The independent variable was red betel leaf extractthe dependent variables is fasting blood sugar levels and ABPI. Using purposive sampling and Lemeshow formula, 70 respondents were selected which were divided into 2 groups, who met specific inclusion and exclusion criteria. Analysis proceeded with a univariate test to determine the frequency distribution of respondent’s characteristics, followed by bivariate analysis using man whitney test. Results: Red betel leaf extract significantly reduced blood sugar levels in patients with type 2 diabetes mellitus with an intervention value of 112.83 ± 15.227 compared to control of 191.46 ± 72277 with a large Cohen's d effect of 0.87 and increased the value of the Ankle Brachial Pressure Index or ABPI in the intervention group by 0.855 ± 0.045 compared to control by 0.674 ± 0.094 with a large d effect of 1.22 These findings suggest that red beet extract has potential as an adjuvant therapy in glycemic control and improvement of peripheral circulation. Conclusion: Betel leaf extract has been shown to be effective in reducing fasting blood sugar levels and improving Ankle Brachial Pressure Index (ABPI) scores in patients with type II diabetes mellitus.
