Abstract

Objective: To prove the success of supportive psychotherapy thatwas provided as a distress therapy on advanced cervical cancer.Knowing the prevalence of distress type of depression in patientwith epithelial cervical cancer, proving the benefits of psychotherapysupportive for distress can decreasing cortisol level in cervicalcancer patient, can assess distress thermometer score, HAM-D17score and scoring incident predictors of distress with depressiontype. Methods: There were 32 subjects from 71 advanced cervical cancerpatients had mild-moderate depression. Then randomizationblocking was performed to determine a subject who entered thetreatment group (n = 16) who got supportive psychotherapy orcontrol group who got common psychotherapy (n = 16). All ofparticipants assessed the distress level with cortisol value, distressthermometer score, and HAM-D17 score before and after they gotsupportive psychotherapy. Results: After the intervention of psychotherapy in the treatmentgroup decreased HAM-D17 score, the average decline 7.53 (SB 3.34).The mean decreasing in the control group was 3.98 (SB 2.85). Thereis a significant difference in mean reduction in HAM-D17 scores ontreatment and control groups with p = 0.003 (p <0.005). There wasdecreasing blood cortisol level in the treatment group amounted to39.43, while the control group there was a drop of 1.59. The reductionof cortisol level in the treatment group and the control has a pvalue0.302. After got supportive psychotherapy, found a decreasingthe average value of the thermometer distress in the treatmentgroup 3.02 and the control group 2.51, with a p value more than0.492.Conclusion: There were 45% of cervical cancer patients in the clinicexperiencing distress disorder with depressive type. The bloodcortisol level could be decreased by giving supportive psychotherapywith a mean decrease of 39.43 nmol/l. There was a significantreduction in the level of depression (HAM-D17 score) of 7.53 pointsand distress thermometer impairment by 3 points after givensupportive psychotherapy. Obtained scoring predictors for theoccurrence of distress type of depression in patients with advancedcervical cancer with a sensitivity of 46.15% and a specificity of89.47%.[Indones J Obstet Gynecol 2018; 6-3: 179-187]Keywords: cervical cancer, cortisol, distress, distress thermometer,HAM-D17 score

Abstract

Objective: To evaluate the relationship between social support given to HIV patients with the incidence of postpartum depression. Method: All HIV patients who had delivery in Dr. Cipto Mangunkusumo hospital and Prof. dr. Sulianti Saroso hospital during the period of October 2012 to March 2013 were enrolled as research subjects. We performed depression screening with the Edinburgh Postnatal Depression Scale (EPDS) and to measure the social support provided by Social Support Questionnaire/Kuesioner Dukungan Sosial (KDS). Statistical analysis was done using Fisher’s exact test, Chi-square and unpaired t-test. Result: We found a total of 34.1% of subjects who experienced postpartum depression, in which 40% did not received proper social support. Considering the demographic characteristics of the subjects who experienced depression, 70% was in age group 20-35 years, 56.7% of subjects had low education level and 70% was primiparous. Conclusion: Social support has a protective effect towards postpartum depression in HIV patients. Abstract

Background: Along with the increasing incidence of primary cancer, metastatic brain tumors are becoming more common. Metastatic brain tumor patients go through a great deal of distress. Therefore, providing adequate standard therapy can alleviate patients’ clinical symptoms and improve their overall quality of life. This study aimed to investigate the role of symptomatic treatments and adjunctive psychotherapy in patients with metastatic brain tumors. Methods: We conducted a randomized, single-blind, parallel design without matching and pragmatic clinical trial at Cipto Mangunkusumo Hospital from August 2019 – June 2020. A total of 30 metastatic brain tumor patients were divided into 2 groups: (1) The treatment group which received a combination of symptomatic and adjunctive psychotherapy, and (2) The control group which was given symptomatic therapy only. The patients’ distress level was evaluated using a distress thermometer (DT). Data analysis was carried out using the Chi-square test, Fisher exact’s, and binominal test. Results: All patients in the treatment group experienced a significant improvement in overall DT scores before and after adjunctive psychotherapy (p=0.01). Symptomatic therapy alleviates headaches and seizures significantly by 100% with or without psychotherapy. Conclusions: The standard symptomatic treatment of metastatic brain tumors can alleviate clinical symptoms in both groups, especially for headaches and seizures. An adjunctive psychotherapy may significantly improve the overall DT.

Abstract

Introduction: Reflective function refers to the capacity to comprehend emotions, beliefs, and behaviors in oneself and others. Mental disorders are frequently associated with deficits in mentalizing capabilities. These deficits compromise an individual’s capacity for self-reflection and understanding of mental states, impeding one’s ability to regulate both mental states and interpersonal relationships. The Reflective Functioning Questionnaire (RFQ) is a practical tool to measure reflective function, but it has not yet been adapted into Indonesian, a crucial step toward addressing the needs of local populations. This study aimed to develop an Indonesian version of the RFQ with robust validity and reliability. Methods: The English RFQ was translated into Indonesian using standard guidelines, including forward and backward translation by certified translators. Content and face validity were assessed through expert panel reviews to ensure cultural and linguistic relevance. Construct validity was evaluated using confirmatory factor analysis (CFA), and reliability testing was conducted using Cronbach’s Alpha. A pilot study with 10 subjects was conducted to refine the instrument, followed by testing with a sample of 100 participants. Results: The final Indonesian RFQ consists of 11 items across two subscales: RFQu and RFQc. CFA indicated a good model fit (χ² = 1.16, p = 0.55, and RMSEA = 0.041). Reliability testing showed strong internal consistency, with Cronbach’s Alpha values of 0.89 for RFQc and 0.87 for RFQu. Conclusion: The modified Indonesian RFQ demonstrated excellent validity and reliability, providing a robust tool for assessing reflective function in clinical and research settings.
