Abstract

Era digital baru ini dapat menguntungkan perusahaan yang beroperasi atau mencari peluang di pasar internasional dalam banyak hal. Era digital menuntut banyak dari mereka untuk menggunakan layanan internet untuk perkembangan bisnis. Perusahaan yang tidak bisa beradaptasi dengan perkembangan zaman seringkali tidak kompetitif. Dalam perkembangannya, kemajuan teknologi komunikasi dan inovasi terkini telah mempengaruhi cara manusia dalam menghadapi transaksi antara produsen dan konsumen. Penelitian ini bertujuan untuk memberikan analisis terkait strategi pemasaran online di era pasar bebas dan globalisasi dengan dukungan teknologi yang berkembang, serta upaya untuk mendeskripsikan konsep strategi pemasaran online dan peluang yang diberikan oleh model pemasaran tersebut. Penelitian ini menggunakan metode kualitatif, dalam jenis penelitian ini temuan tidak diperoleh melalui prosedur statistik atau perhitungan lainnya. Hasil penelitian ini mengarah pada pernyataan bahwa komunikasi pemasaran online dan bentuk barunya memberikan cakupan yang lebih luas, sehingga mempromosikan cakupan iklan yang lebih baik. Melalui pemasaran online, pengembangan merek dapat menjadi lebih baik daripada model pemasaran tradisional. Situs web yang dirancang dengan baik dengan informasi berkualitas tinggi dapat memenuhi kebutuhan pelanggan dan menciptakan lebih banyak peluang untuk memberikan nilai yang signifikan bagi pelanggannya. Kata kunci: Marketing, digital, teknologi, internet.

Abstract

Penelitian ini bertujuan untuk mengetahui persepsi guru tentang supervisi akademik dan self-efficacy guru dalam implementasi Kurikulum Merdeka pada Sekolah Menengah Pertama Negeri di Kecamatan Sidoarjo serta menguji pengaruh diantara keduanya. Penelitian ini menggunakan metode kuantitatif dengan teknik analisis data uji regresi linier sederhana. Sampel penelitian ini merupakan guru-guru dari 6 Sekolah Menengah Pertama Negeri di Kecamatan Sidoarjo sebanyak 74 responden dengan menggunakan teknik random sampling. Hasil penelitian ini menunjukkan bahwa (1) persepsi guru tentang supervisi akademik pada Sekolah Menengah Pertama Negeri di Kecamatan Sidoarjo memiliki kategorisasi tinggi. (2) Self-efficacy guru Sekolah Menengah Pertama Negeri di Kecamatan Sidoarjo memiliki kategorisasi tinggi. (3) Terdapat pengaruh positif dan signifikan persepsi guru tentang supervisi akademik terhadap self-efficacy guru.

Abstract

Emphysematous cystitis (EC) is a rare and potentially life-threatening infection of the bladder wall characterized by the presence of gas. It predominantly affects elderly females with diabetes mellitus. While the clinical presentation can vary, it commonly involves symptoms of urinary tract infection. This case report describes an unusual presentation of EC in a geriatric female complicated by culture-proven urosepsis. A 66-year-old female with a history of type 2 diabetes mellitus presented to the emergency department with fever, dyspnea, and tachypnea. She reported suprapubic pain, dysuria, and foul-smelling, murky urine for two days. Initial laboratory findings revealed leukocytosis, hyperglycemia, and elevated procalcitonin levels, indicating sepsis. Computed tomography (CT) of the abdomen and pelvis revealed air foci within the bladder wall, consistent with EC. Blood and urine cultures grew Escherichia coli resistant to multiple antibiotics but susceptible to amikacin and gentamicin. The patient was treated with intravenous amikacin and showed significant clinical improvement, with subsequent negative urine cultures. In conclusion, our study highlights a rare presentation of emphysematous cystitis complicated by culture-proven urosepsis in a geriatric female with multidrug-resistant Escherichia coli. Prompt diagnosis using CT imaging and appropriate antibiotic therapy based on culture sensitivities led to a successful outcome. This case underscores the importance of considering EC in elderly diabetic patients presenting with sepsis and urinary symptoms, even with atypical features.

Abstract

Background: The optimal surgical approach for repairing a vesicovaginal fistula (VVF) remains a subject of significant clinical debate. Both transabdominal and transvaginal techniques have distinct advantages and disadvantages, leading to a dilemma in surgical decision-making. This systematic review and meta-analysis aims to provide a comprehensive, evidence-based comparison of the two approaches, focusing on surgical success, complications, and perioperative outcomes. Methods: This review was conducted and reported in accordance with the PRISMA 2020 guidelines. A systematic search of PubMed, Scopus, and Web of Science was performed for comparative studies published between January 2015 and August 2025. We included studies directly comparing transabdominal and transvaginal VVF repair. The primary outcome was surgical success (fistula closure). Secondary outcomes included overall complications, mean operative time, and length of hospital stay. A random-effects model was used for meta-analysis to calculate pooled Odds Ratios (OR) and Mean Differences (MD) with 95% Confidence Intervals (CI). Results: From 1,284 articles identified, seven comparative studies involving 678 patients (335 transvaginal, 343 transabdominal) were included. The meta-analysis revealed no statistically significant difference in surgical success rates between the transvaginal and transabdominal approaches (OR 1.12, 95% CI 0.68,1.85, p=0.65; I²=21%). However, the transvaginal approach was associated with a significantly lower rate of overall complications (OR 0.45, 95% CI 0.28,0.73, p=0.001; I²=0%). Furthermore, the transvaginal approach demonstrated significantly shorter mean operative times (MD -58.45 minutes, 95% CI−75.60,−41.30, p<0.00001; I²=88%) and a shorter mean length of hospital stay (MD -3.15 days, 95% CI −4.01,−2.29, p<0.00001; I²=92%). Conclusion: While both surgical approaches yield comparable fistula closure rates, the transvaginal technique offers a superior safety and efficiency profile, with significantly fewer complications, shorter operative times, and reduced hospitalization. These findings suggest that the transvaginal route should be considered the preferred approach for anatomically suitable fistulas, though surgeon expertise and fistula characteristics remain paramount in resolving the VVF repair dilemma.
