Abstract

Objective: The aim of this study is to study the effect of preoperative Ketorolac on postoperative pain after transurethral prostatectomy at Sardjito Hospital Yogyakarta. Material & Methods: This is a cross sectional, analytic, prospective study. We divided benign prostatic hyperplasia patients, from October 2011 until February 2012 into two groups. Group I was given Ketorolac 30 mg intravenously twice daily starting one day before operation and in the morning before procedure. Group II did not receive Ketorolac preoperatively. Patients underwent transurethral operation with spinal anesthesia. We assessed postoperative pain at 24 hours using Visual Analogue Scale (VAS). Results: Twenty four patients were included in this study with mean age 66 years old with the youngest 45 years old and the oldest 80 years old (SD 8,77744). Group I (17 patients), VAS score 1 to 7 with median 3 and mean 3,4118 (SD 1,66053). Group II (7 patients) VAS score 1 to 8 with median 3 and mean 3,8571 (SD 2,8357). We count it using Mann-Whitney U (p = 0,951). Conclusion: There is no significance difference in post-operative pain after transurethral prostatectomy with preoperative ketorolac administration. Keywords: Ketorolac, bupivacaine, visual analogue scale, transurethral prostatectomy.

Abstract

This qualitative research, conducted through observation, interviews, and documentation, aims to examine and explore how the sorogan method is implemented, what influences its implementation, and what impacts occur after implementing it. The results show that the Fajrussa’adah Islamic boarding school is one of the Salaf Islamic boarding schools that uses the yellow book and the sorogan method as a source and method in its learning. The implementation takes place three days a week and is held in the evening after the Isha prayer. Several supporting factors include strong student motivation and the presence of a teacher. Several inhibiting factors include students' fatigue after attending formal school outside the boarding school and many feeling sleepy because classes are held at night. Nevertheless, the implementation of sorogan has a significant impact on students in terms of reading and understanding the yellow book and has an impact on their daily lives, making them more mature in preparing things.

Abstract

Introduction: Tuberculosis (TB) is caused by Mycobacterium tuberculosis (MTB) and now represents a significant global health concern, with an estimated 10 million people infected with TB in 2022, equivalent to 133 incident cases per 100,000 population. Indonesia faces a substantial burden. The World Health Organization (WHO) estimates 969,000 TB cases, with 717,941 confirmed cases reported. Extrapulmonary TB, contributing to 15% of TB cases, affects various areas such as bones, pleura, lymph nodes, genitourinary system, peritoneum, and meninges. Tuberculous epididymo-orchitis, a rare type of extrapulmonary TB, is characterized by MTB infection in the epididymis or testis. Case: A 31-year-old male was presented to the Emergency Department of Budi Rahayu General Hospital, Pekalongan, with a fever, bitter taste, body aches, and cough. Initially hospitalized for five days and discharged upon improvement of symptoms, the patient returned with fatigue, cough, and black stools. After being admitted under internist and pulmonary specialist care due to a suspected left lung TB apical lesion, biomolecular testing (GeneXpert) was performed. Consultation with a urologist revealed scrotal pain, prompting further tests including urine acid-fast bacilli (AFB) examination, testicular ultrasound, and aspiration culture. The patient was diagnosed with TB epididymo-orchitis and treated with a Category 1 TB regimen. Conclusion: TB epididymo-orchitis is a rare case requiring thorough diagnostic evaluation encompassing anamnesis, physical examination, and diagnostic procedures. Early detection and appropriate management are vital in combating this challenging disease.
