Abstract

Management in patients with type 2 diabetes mellitus (DM) is aimed diabetic control required antidiabetic drugs(OAD). The aim of this study was to know data about the profile of diabetics control in patients with type 2 diabetesmellitus followed in department of endocrinology Arifin Achmad General Hospital Pekanbaru. The type of researchwas descriptive. The samples are 96 people included all patients with type 2 DM according to inclusion criteria. Theidentity patients and patients knowledge were determined using questionnaire, used of antidiabetics drugs and diabeticcontrol were detemined using medical record. The characteristics of patients with type 2 DM were most at group ofage 50-59 years (45,8%), female sex (58,3%), duration of DM 1-5 years (52,1%), complication of diabetic neuropathy(25%) and frequency of the Doctor once a month (64,6%). The used OAD predominant oral antidiabetic drugs (83%)combinations sulfonylureas and biguanid (73,7%), combinations oral antidiabetic drugs with insulin (14%) arecombinations biguanid with long acting insulin (69,2%) and intensive insulin (3%) are long acting insulin with rapidacting insulin (66,6%). Diabetic control of patients with type 2 DM are fasting blood glucose, post prandial 2 hoursblood glucose, total cholesterol, LDL and HDL was bad (68%, 74%, 46,9%, 61,5% and 70,8%). This researchshowed that used of antidiabetic drugs was not satisfactory of diabetic control.

Abstract

Diabetes mellitus (DM) is a group of metabolic disease which characterized byhyperglycemia, caused by insulin secretion or insulin activity disorder or both.Uncontrolled DM could cause many complications, one of them is diabetic ulcer. Thesample was 39 patients using total sampling. In conclusion,characteristics of respondentsby sex diabetic ulcer patients obtained 56.42% were females, age group <60 yearsobtained 82%, and duration of suffering DM obtained 48.72% 0-5 years. Distribution ofdiabetic ulcers based on fasting blood sugar obtained 93.93% is bad.. Diabetic ulcersdistribution based on the location of 47.91% was obtained at toes, feet and back legsrespectively 16.67%, 10.41% ,other areas and heel respectively 8.33%. Distribution ofdiabetic ulcer based on Wagner criteria was obtained 33.3% Grade 4, Grade 3 obtained25.6%, 23.1% obtained Grade 5, Grade 2 obtained 10.3%, 7.7% obtained Grade 1 andGrade 6 obtained 0%. Distribution of diabetic ulcer based on length of stay obtained63.15% were> 7 days, 3-7 days obtained 28.94%, and <3 days gained 7.89%. Distributionof diabetic ulcer based on therapy obtained 89.7% were non-surgical, and surgical therapyobtained 10.3%.Keywords: DM, diabetic ulcer, profile.

Abstract

Diabetic ulcer is chronic complication of diabetes melitus shown as connective tissues ulceration and destruction on lower limbs caused by uncontrollable hyperglycaemic of Diabetes melitus patient that lead into peripheral angiopathy and neuropathy so that bacterial infection trauma will easily occur. This research’s goal is to find out bacterial pattern and antibiotic resistance of second grade diabetic ulcer patients that have been hospitalized in General Hospital Arifin Achmad on 2012. 23 comprehensive medical records have been used in this descriptive and retrospective methode of research. From distribution of age category has been found 40-59 years old as the highest percentage (87%). Based on gender has been found female respondents (78,3%) and male respondents (21,7%). Most of patients have been suffered Diabetes melitus for less than 5 years (60,9%) and allowed to went home after treatment (82,6%). Most commonly bacteria that found based on bacterial culture and resistance test was Acinetobacter baumanii (34,8%), Klebsiella pneumoniae (26,2%) and Escherichia coli (17,4%). Amoxicillin and ampicillin have lowest sensitivity (0%), followed by trimethoprim/sulfametoxazole (17,4%) and cefotaxime as well as ciprofloxacin was 21,7%. The highest sensitivity antibiotics was meropenem (100%), imipenem, amikacin, and colistin (95,6%), followed by ertapenem (91,3%).Keywords: diabetic ulcer, bacterial pattern, resistance test, sensitivity, antibiotic

Abstract

Diabetes mellitus (DM) is a group of metabolic diseases, the characteristic is hyperglycemia that causes by abnormal of insulin secretion, insulin action or both of it. Insulin is a therapy of type 2 DM. Data of International Diabetes Federation showed that using insulin therapy in type 2 DM patient in Indonesia was classified low. This study aims to determine the description of the achievement insulin terapy level in type 2 DM patient’s. Design of this study is retrospective descriptive is done with and collect data from medical record to 114 type 2 DM patients who received combination oral antidiabetic agents with insulin therapy and intensive insulin therapy. The result of this research were found that combination oral antidiabetic agents with insulin therapy usage are higher than intensive insulin therapy is 77,2%. The most used combination therapy was combination long acting insulins with biguanids and sulfonylureas is 14,8%. The most used intensif insulin therapy was long acting insulin and rapid acting insulin is 38,4%. Achievement of therapeutic targets DM is 69,8% fasting blood glucose levels and 82,8% 2-hour postprandial blood glucose levels in bad catagory, 46% Triglycerides levels in avarege catagory, 64,2% HDL cholesterol in low catagory, 61,8% LDL cholesterol in bad catagory, 34% total cholesterol in bad catagory and 50% blood preassure in avarege catagory. Incidence of hypoglycemia is 7%. Conclusion of this study is the avhievement insulin therapy in type 2 diabetes mellitus patients is low catagory.
