Effect of Ovarian Stimulation with Recombinant FSH for In Vitro Fertilization (IVF) on Anti Müllerian Hormone (AMH) Levels as an Early Marker of Ovarian Reserve Hapsari, Kartika

Abstract

Objective: To analyze the effect of ovarian stimulation with recombinant FSH on AMH levels as a marker of ovarian reserve in patients undergoing IVF. Method: This study is an intervention study which compares AMH levels pre- and post-ovarian stimulation with recombinant FSH in IVF participants to determine the effect of ovarian stimulation on ovarian reserve. This study was conducted at Dr. Cipto Mangunkusumo Hospital, Jakarta, from January to July 2010. AMH levels measured are AMH levels pre- and post-stimulation taken on the day of hCG evaluation. Result: Of 56 patients undergoing IVF treatment, 20 subjects were eligible for study. The mean age of the patients was 35. 3 ± 4.0 years, the mean duration of infertility 9.1 ± 5.7 years, the mean BMI 21.9 ± 3.4 kg/m2, the median AMH level pre-stimulation was 4.0 ng/ml and the total dosage of FSH used was 2747.5 ± 1076.3 IU/ day, the mean duration of ovarian stimulation was 9 ± 3 days. Of the 20 subjects recruited, 11 patients (55%) had a decrease in AMH levels, and the remaining 9 patients (45%) did not. Data analysis showed that the decrease in AMH levels was not statistically significant (p = 0.295) [AMH1; median 4.0 ng/ml (range 2.2 - 5.9) and AMH2; median 3.2 ng/ml (range 1.6 - 4.8)]. Conclusion: AMH levels decrease following ovarian stimulation with recombinant FSH is not statistically significant, so it can be concluded that ovarian stimulation in IVF has no effect on ovarian reserve. Abstract

Building Information Modeling (BIM) is a technology-driven approach that integrates and manages information from all aspects of a construction project within a comprehensive 3D model. Using BIM-based planning makes the process more efficient, helps minimize over-budget issues, and simplifies volume calculations. There are various BIM software tools such as Autodesk Revit, Tekla Structures, and others, each with its strengths and weaknesses. In this study, the researcher analyzes the Quantity Take-Off (QTO) of the main column reinforcement using Autodesk Revit and Tekla Structures. The results of the rebar QTO from the outputs produced by both software tools showed a total deviation of 0%, indicating that both were equally accurate in generating QTO. This suggests that regardless of the software choice, the accuracy in modeling and managing 3D data remains reliable if the modeling process is done correctly and precisely. Furthermore, QTO calculations using BIM software proved to be more effective, with a result difference of 0.132% compared to conventional methods, as BIM facilitates data processing and automation of changes, directly linked to the 3D model. This ensures that any change in one task automatically adjusts related tasks, thus speeding up the planning and execution process in construction projects.

Abstract

Objective: Ovarian cancer is the fifth leading cause of cancer-related death in women, due to late diagnosis and limited screening methods. Chondroitin Sulfate-E (CS-E) has shown potential as biomarkers. This study aims to evaluate CS-E expression in epithelial-type benign and malignant ovarian tumors and its potential as a biomarker using QuPath software.Methods: This observational analytic study used a cross-sectional design. Samples were selected based on histopathology of patients with epithelial-type benign and malignant ovarian tumors from surgeries in 2023. Immunohistochemistry using the GD3G7 antibody was performed to detect CS-E expression in tumor tissues preserved in paraffin blocks at Dr. Hasan Sadikin General Hospital Bandung. Expression was quantified using QuPath software. Statistical analysis used the Mann-Whitney and t-test. Result: No significant difference in CS-E expression was found between malignant and benign tumors (p = 0.492). Demographic factors (age, BMI, menopausal status, and parity) showed no significant differences between groups. Conclusion: CS-E expression has not yet demonstrated potential as a biomarker to distinguish between benign and malignant ovarian tumors.Perbandingan Ekspresi Chondroitin Sulfate-E pada Tumor Ovarium Jinak dan Ganas Tipe Epitel Abstrak Tujuan: Kanker ovarium merupakan penyebab kematian kelima terbanyak terkait kanker pada wanita yang disebabkan oleh keterlambatan diagnosis dan keterbatasan metode skrining. Chondroitin Sulfate-E (CS-E) menunjukkan potensi sebagai biomarker. Penelitian ini bertujuan mengevaluasi ekspresi CS-E pada tumor ovarium jinak dan ganas tipe epitelial serta menilai potensinya sebagai biomarker menggunakan perangkat lunak QuPath. Metode: Penelitian analitik observasional ini menggunakan desain potong lintang. Sampel dipilih berdasarkan hasil histopatologi pasien dengan tumor ovarium jinak dan ganas tipe epitelial dari operasi tahun 2023. Pemeriksaan imunohistokimia dengan antibodi GD3G7 dilakukan untuk mendeteksi ekspresi CS-E pada jaringan tumor yang diawetkan dalam blok parafin di RSUP Dr. Hasan Sadikin Bandung. Ekspresi dianalisis secara kuantitatif menggunakan perangkat lunak QuPath. Uji statistik yang digunakan adalah Mann-Whitney dan uji t. Hasil: Tidak ditemukan perbedaan bermakna dalam ekspresi CS-E antara tumor ganas dan jinak (p = 0,492). Faktor demografis (usia, indeks massa tubuh, status menopause, dan paritas) juga tidak menunjukkan perbedaan bermakna antar kelompok.Kesimpulan: Ekspresi CS-E belum menunjukkan potensi sebagai biomarker untuk membedakan antara tumor ovarium jinak dan ganas.
