Abstract

Background: Natural resistance associated macrophage protein (NRAMP) is a proton/ divalent cation transporter which play a role in iron trafficking in the phagosomes. Variations in NRAMP1 gene have been reported to be associated with susceptibility to tuberculosis (TB) because Mycobacterium tuberculosis (MTb), the causative agent of TB, compete with its host to uptake iron for its metabolism. The study aimed to describe the polymorphism of NRAMP among TB patients in Kupang, East Nusa Tenggara.Methods: This is a case-control study, cases were pulmonary TB, new patients, aged 15-55 years with sputum smear positive for acid fast bacili. Control were surrounding neighbours without symptoms and history of TB. All demographic information and blood sample were taken for polymorphism. PCR/RFLP method was performed to explore whether single nucleotide polymorphism D543N of NRAMP1 gene is associated with susceptibility to TB.Results: The study involved 64 pulmonary TB patients and 51 healthy controls. We observed a significant different in the distribution of NRAMP1 genotypes frequencies between TB patients and healthy controls (p = 0.014). Moreover, D543N polymorphism gave significant association only in male subjects. Though the numbers of the subjects are limited, D543N NRAMP1 polymorphism in endemic region in Kupang, the eastern part of Indonesia, seems to be associated with the susceptibility to TB. This is in contrary to studies reported in other part of Indonesia: i.e from west part (Jakarta, Bandung) and central part (Makassar). The population from Kupang may similar genetic background as African population, as Mycobacterium infected in population from Kupang is mostly Mycobacterium africanum. Increasing the number of subjects may enhance the power of the study and possibility to meet Hardy-Weinberg Equilibrium to detect the true associations of this polymorphism in susceptibility to TB.Conclusion: There was a significant difference of polymorphism NRAMP1 which more pronounced among male subjects, however this has not yet fulfilled the Hardy-Weinberg equilibrium. Abstract

It was a great news that during the President election campaign, both President and Vice President candidates addressed a stunting problem during the debate. It shows that stunting is placed as national priority of nutrition problems. Stunting is part of a wider nutrition crises in Indonesia: wasting, anemia and double-burden of malnutrition. According to the 2018 National Health Survey (RISKESDAS), 30.8 percent of Indonesian children under 5 years of age were stunted (almost 8 million children), 17.7 percent were underweight, 10.2 percent were wasted (low weight-for-height).  Although the prevalence of stunting declined significantly from the 2013 (37.2%), the stunting rates remain unacceptably high. A child that is wasted faces double the risk of mortality associated with stunting. Even, a child that is both stunted and wasted faces a higher risk. Frequent episodes of wasting increase the risk of stunting.

Abstract

ABSTRACTThe objective of this study was to assess the micronutrient and nutritional status among female adolescents having low birth weight history and normal birth weight history. Research design for this particular research was nested cohort in community level. Subjects was chosen by simple randomization, based on data collected by Frontiers for Health Foundation and Padjajaran University, Bandung. The measurement of micronutrients status (Hb, retinol plasma, and urinary iodine excretion), anthropometric, history of disease, semi quantitative food frequency questionnaire was collected to make comparison between female young adolescent with low birth weight and normal birth weight history. The prevalence of iron deficiency anemia (IDA), vitamin A deficiency (VAD), and iodine deficiency among all subjects were 9.9%, 12.9%, and 40.7%, respectively. The mean concentration of blood hemoglobin, plasma retinol and iodine excretion were not significantly different between low birth weight (LBW) and normal birth weight (NBW) group. Nutritional status, disease during the last three months, current nutritional status, food intake and onset of menstruation were not significantly different between group. In conclusion, micronutrient status, nutritional status, food intake and morbidity status in female adolescents with LBW and NBW history were not different.Keywords: iodine, iron, low birth weight, normal birth weight, vitamin AABSTRAKTujuan penelitian ini adalah mengukur status mikronutrien dan gizi remaja putri yang memiliki riwayat berat badan lahir rendah dan berat badan normal. Desain penelitian ini adalah nested cohort retrospective untuk tingkat komunitas. Subjek penelitian dipilih berdasarkan randomisasi sederhana, berdasarkan data cohort yang dikumpulkan oleh Yayasan Frontriers for Health dan Universitas Padjajaran, Bandung. Pengukuran status mikronutrien (Hb, plasma retinol, dan kadar iodium dari ekskresi urin), pengukuran antropometri, riwayat penyakit yang diderita dan pengumpulan data mengenai asupan makanan dilakukan perbandingan antara remaja putri yang memiliki berat badan normal dan berat badan lahir rendah. Prevalensi anemia kekurangan zat besi, kekurangan vitamin A dan kadar iodium dari ekskresi urin pada semua subjek adalah 9,9%, 12,9%, dan 40,7%, secara berurutan. Konsentrasi rata-rata dari hemoglobin darah, plasma retinol dan kadar iodium dari ekskresi urin tidak berbeda secara nyata antara remaja putri dengan berat badan lahir rendah dan berat badan normal. Status gizi, penyakit yang diderita selama tiga bulan terakhir, asupan makanan, dan usia mulai mengalami menstruasi tidak berbeda secara nyata antara remaja putri dengan berat badan normal dan berat badan rendah. Status mikronutrien, status gizi, asupan makanan, dan status morbiditas pada remaja putri dengan berat badan lahir rendah dan berat badan normal tidak terdapat perbedaan yang nyata.Kata kunci: BBLN, BBLR, iodium, vitamin A, zat besi
