Abstract

Background: Early posypartum haemorrhage (PPH) is a crucial problem in maternal and newborn care. The simulation-based learning has an important role to improve knowledge, practical skills, and attitude in realistic case-based scenarios. Purpose: This study aims to examine the effect on using simulated scenario in the simulation laboratory on nursing performance of early postpartum hemorrhage of nursing students. Methods: The quasi-experimental research, one group pretest-posttest design was used to study with the nursing students from the Faculty of Nursing, Naresuan University in Academic Year 2019 during the period of December 1, 2019 to January 31, 2020. The subjects of thirty nursing students were selected by purposive sampling. The instruments were the the ability in nursing of early postpartum hemorrhage through the questionnaires which consists of the performance test and satisfaction before and after the early PPH simulation program. The content of scenario consists of the definition, etiology, risk factors, signa and symptoms, treatment, holistic nursing care, and continuous care. The data were analysed as the descriptive: numbers, percentages, frequency, mean and standard deviation. The paired t-test was used to compare the outcome between pre-test and post-test. Results: The data were analysed by t-test. The results illustrated that the students who learned through simulated scenario on performance in nursing care of women with early postpartum hemorrhage had, after learning, higher performance in nursing care of women with early postpartum hemorrhage than that before learning at the .01 level of significance. The students were satisfied learning by using the simulation program in the high level (x̄ = 4.26, S.D. = 0.54). Conclusion: The simulation-based learning improves the core competencies and performances of nursing students for management women with early postpartum hemorrhage as knowledge, potential clinical care skills, and attitude. It will be useful to implement for nursing care women regarding early postpartum hemorrhage in the real situations.

Abstract

Introduction: Iron deficiency anemia (IDA) is a global health problem. The prevalence of anemia in pregnancy worldwide is nearly half of pregnant women. It impacts on women and offspring outcomes during pregnancy, intrapartum and postpartum period associated with increasing rate of preterm labor, pregnancy induced hypertension, low birth weight, perinatal death including postpartum hemorrhage, postpartum infection, unsuccessful rate of exclusive breast feeding, and postpartum depression. Inadequate iron intake, maternal physiological changes during pregnancy, and bleeding were indicated as common causes of IDA in pregnancy. The objective was to explore the experiences of pregnant women regarding IDA.Methods: A total of eighteen women between 16-36 weeks gestation participated in the qualitative research. Women were selected by purposive sampling according to inclusion criteria to in-depth interviewed at antenatal care clinic, Watbot hospital, Phitsanulok, Thailand.Results: Thematic analysis of the qualitative interviews identified four main themes: iron-deficiency anemia in pregnant as a normal pregnancy; concern on food rather than hematocrit (Hct) level; maternal instinct in healthy baby; and low socioeconomic as a main obstacle.  Conclusion: The findings illustrated to enhance better understanding the nature, attitude, knowledge, perception, and behavior of pregnant women on IDA, facilitators to support women for healthy behavior, and barriers to IDA in pregnancy based on Thai context.

Abstract

Background: Iron deficiency anemia in pregnant women is a crucial global health problem. Iron deficiency anemia is impacted on health during antenatal, labor and postpartum period. Although the national policy provides the preventive and solving the maternal anemia, the statistics of maternal anemia  are still high. Purpose: The aim of the study is to develop a health promotion program that is suitable for maternal anemia. Methods: Qualitative research was used to develop the health promotion program  on maternal anemia through in-depth interviews. Ten pregnant women who had hematocrit less than 33 volume percentages or hemoglobin less than 11 g/dl and five healthcare professionals were selected by purposive sampling. A total of fifteen participants were interviewed based on the semi-structured questionnaire for 30-45 minutes per case at the antenatal care clinic, Watbot hospital, Phitsanulok, Thailand. The period of the study was six months from the first of August 2022 until the end of January 2023. Results: Thematic analysis was used to analyze the data and identified three main themes: 1) encouragement the attitude of self-care during pregnancy; 2) accessibility of the program ; and 3) practical use of the program  and integrating the program  based on the context of pregnant women and healthcare professionals. Conclusion: The health promotion program was developed suitable for maternal anemia. The tailored program should be tested in terms of feasibility, accessibility, and practical use. It will support pregnant women with anemia, develop a key  performance index of maternal and child health, decrease risks and complications, and promote maternal and child health based on the various contexts.
