Abstract

Background: Magnetic Resonance Imaging (MRI) examination of the cervical spine axial slices with T2 weighted flow is often contained artifacts that interfere image result, so it uses flow compensation technique. Axial slices of T2 weighted may use TSE or GRE sequences. This research aims to determine differences in MRI image information of cervical spine axial slices using flow compensation between T2 weighted turbo spin echo (TSE) and gradient echo (GRE) sequences and to get better image information between that both sequences. Methods: This research was a quantitative study with an experimental approach. Data were collected from May to June 2016 in Radiology Installation of Haji Surabaya Hospital using a questionnaire to three respondents. Data were analyzed by statistical tests with SPSS 16 software using Wilcoxon test to determine differences level in image information between T2 weighted TSE and GRE sequences, mean rank value from Wilcoxon test and crosstabulation test to get better image information. Results: There were differences in MRI image information of cervical spine axial slices with using flow compensation between T2 weighted TSE and GRE sequences with p-value was 0.005 (p 0.05). This difference was due to the differences of parameter characteristics between both sequences and T2* GRE was more sensitive to flow to the organ than the TSE. Better MRI image information of cervical spine axial slices using flow compensation was T2* GRE sequence shown by the mean rank 5.50.T2*. GRE was better than T2 TSE because it could show the anatomy of the cervical clearly and informatively, the anatomy of the spinal cord, CSF, neural foramen, intraforaminal structure, corpus vertebrae, disc and the minimum artifacts. While on the anatomy of the facet joint was no significant difference between the TSE and GRE sequences. Conclusion: There were differences in the MRI image information of cervical spine axial slices using flow compensation between T2 weighted TSE and GRE sequences. Better image information was T2* GRE sequence.

Abstract

Background: The imaging for vein vessels or Magnetic Resonance Venography (MRV) has own role and advantages on MRI examinations, specifically for evaluating intracranial blood vein circulation. MRV generally uses Phase Contrast (PC) method and  PC-MRV imaging, there is an  important parameter to be considered, its velocity encoding or VENC. VENC is selected before the examination and has to be adjusted  the anticipated blood flow velocity in the examined organs. The right selection of VENC will result in optimum intracranial vessels images and accurate diagnose. The aim of this study is to figure out if there is significant effect of VENC on anatomical information of brain MRV and to find out which VENC the best  intracranial veins demonstrates. Methods: This research was a quantitative study with quasi – experimental approach. Data were obtained from five healthy volunteers who were scanned by using a 3 Tesla MRI device in Radiology Department Siloam Lippo Village Hospitals. Each volunteer was scanned with five VENC variations: 10 cm/s, 15 cm/s, 20 cm/s, 25 cm/s, and 30 cm/s. The MRV images were assessed by two radiologist as the respondent. The Data were analyzed by simple linear regression test and Friedman test.Result: The results showed that there was significant effect of VENC on anatomical information of brain MRV, with significant value below 0,001 (p value 0,05). Mean rank on Friedman test showed that the best VENC to demonstrate intracranial veins was 25 cm/s.Conclution: There was a significant effect of VENC on anatomical information of brain MRV with the VENC of 25 cm/s gave the best image of intracranial veins in general.

Abstract

Backgroud: Examination of the abdomen CT scan is often done by using standard protocol, meanwhile the actual parameter can be modified according to local needs considering image quality and radiation dose based on Karabulut and Ariyuek (2016). Abdomen CT Scan by GE 16 slices unit in Radiology Instaallation of Dr. Saiful Anwar Malang Hospital, using exposure factor of 120 kV, 234 mAs and the value of the CTDI dose was 53.04 mGy. While the BAPETEN’s reference of CTDI value, a CT scan of abdomen was 25 mGy.  This study aims to determine the changes of the value of kV and mAs to the image quality and the radiaton dose in the abdomen MSCT examination on abdominal tumor case in Radiology Installation of Dr. Saiful Anwar Malang Hospital. Methods: This research was a quantitative research with an experimental approach. The data were collected from three variations of tube voltage (kV) ie 100 kV, 120 kV and 140 kV and three variations of tube current value and time (mAs) ie, 180 mAs, 195 mAs, and 210 mAs. Radiographs was evaluated by three radiology physicians. Data were analyzed by scoring method of respondent’s assessment to assess MSCT image quality of abdominal tumor, while the radiation dose was obtained by CTDI recording. Results: The results showed that there was influence of tube voltage variation to image quality of abdominal tumor using MSCT unit. The higher kV used, the higher image quality resulted. From the calculation of the percentage from the assessment, the highest score of variation was at 140 kV, while the lowest score of variation was at 100 kV. Based on the recording CTDI radiation dose on the monitor, found that the higher value of kV, the higher radiation resulted. The mAs variations influenced the image quality of abdominal tumor using MSCT unit. Acoording to the percentage of the assessment, the highest score of variation found in 210 mAs, and the lowest score of variation found in 180 mAs. Based on the recording of radiation dose on the monitor, the higher mAs value, the higher radiation dose using MSCT unit. It was recommended to use 195 kV and 120 mAs for MSCT examination of abdominal tumor in Radiology Installation of Dr. Saiful Anwar Malang Hospital. Conclusion: There was influence of kV and mAs variation to anatomical image clarity and radiation dose of abdomen MSCT examination

Abstract

Background : Many hospitals in the examination of pedis AP projection use 0º or perpendicular. It is not based on the theory. So that, it is done research about the variation of direction angle of AP projection in pedis examination toward anatomy information. The examination is aim to find how big the direction angle toward anatomy information which is result by examination pedis AP projection and angle which is result by optimal anatomy information. Methods : The research which is used descriptive quantitative research by experiment approach. The research is done by pedis phantom which eksposes four times with same expose factor with difference angle variation 0º, 5º, 10º and 15º cephalad and quenstionnaire filling. The research using the independent variable is the direction angle on the examination pedis AP projection and the dependent variable is anatomical information on the radiograph pedis AP projection. The quenstionnaire is filled by three respondents, they are Radiologist. The scoring result of the respondents processes and present in table and graphic to describe and analyse for getting conclusion. Results : From the scoring result, it is seen the influence of angle toward pedis anatomy information difference. When use of the angle 0º and 5º cephalad show very clear navicular. While the angle of 10º cephalad show open joint space between medial cuneiform and the intermediate cuneiform and a very clear  cuboid. The angle of 15º cephalad shows a very clear tarsometatarsal joint. And then, the angle which is yield optimal anatomy information in examination pedis AP projection is the  angle of 10 º cephalad. Conclusion : The radiographic technique of the AP projection pedis with a variation of 0º, 5º, 10º and 15º cephalad produces different anatomical information. At angle 0º and 5º showing very clear navicular, 10º cephalad revealed joints between the medial cuneiform and intermediate cuneiform and cuboid and 15º cephalad shows the tarsometatarsal joint very clearly. The optimal angle in generating anatomical information on the AP projection examination is 10 º cephalad.
