Abstract

Menurut Kementrian Kesehatan RI tahun 2020 jumlah angka kematian ibu (AKI) di Indonesia masih cukup tinggi sebanyak 4.627 kasus kematian ibu. Jumlah ini menunjukan peningkatan dibandingkan tahun 2019 sebesar 4.221 kasus kematian ibu dan angka kematian bayi (AKB) sebanyak 72,0% atau 20.266 kasus (Profil Kesehatan RI, 2021). Puskesmas Bantarkawung salah satu naungan Kabupaten Brebes yang berada di Jawa Tengah menyumbang AKI sebanyak 2 kasus dan AKB sebanyak 2 kasus pada tahun 2020. Salah satu upayanya dengan melakukan asuhan kebidanan komprehensif dengan pendekatan continuity of care yang dilakukan mulai pada ibu dalam masa kehamilan, persalinan, nifas, bayi baru lahir dan KB. Peneliti mengimplementasikan asuhan kebidanan secara komprehensif menggunakan pendokumentasian Varney dan SOAP. Penelitian ini jenis kualitatif dengan pendekatan stady kasus dengan wawancara terhadap 1 informan utama yaitu Ny. R. Informasi tambahan 1 bidan koordinator, 1 keluarga pasien dan 1 informan triangulasi yaitu seksi Kesehatan Keluarga Dinas Kesehatan Kabupaten Brebes. Pada kunjungan I kehamilan ditemukan masalah yaitu kekurangan energi kronik (KEK) dengan LILA 22,5 cm dan telah diberi asuhan tentang gizi seimbang tinggi kalori dan tinggi protein. Setelah dilakukan 3 kali kunjungan mengalami kenaikan LILA 1,3 cm sehingga menjadi 23,8 cm. Proses persalinan tidak ditemukan masalah dan asuhan persalinan dilakukan dengan 60 langkah APN serta asuhan bayi baru lahir dengan hasil normal juga. Pada masa neonatus ditemukan masalah ikterus fisiologis sedangkan masa nifas normal dan Ny.R memutuskan menggunakan KB suntik 3 bulan

Abstract

Risk factors during pregnancy can increase complications including gestational hypertension which can contribute to MMR.Maternal mortality according to WHO (2021) is 329,000 people, in Indonesia (2021) there are 7,389 cases, in Central Java (2022) there are 864 cases, in Brebes Regency (2022) there are 50 cases and at the Bumiayu Health Center which is a shade of Brebes Regency also contributes 2 cases. The purpose of this study is to provide midwifery care for pregnant women, maternity, newborns, postpartum and family planning comprehensively. The method in this study used descriptive qualitative with a case study approach. Data collection techniques with interviews, measurements, physical examination and documentation. The main informant is Mrs. S with additional informants namely husband, family, midwife and Dr. Sp.OG. Based on the results of pregnancy, childbirth, newborn, postpartum and family planning. Found problems during pregnancy, namely Mrs. S with blood pressure 140/103 mmHg, negative urine protein which is classified as gestational hypertension.

Abstract

Quick Wins blood service program is a program to support nawacita in the health sector in order to reduce maternal mortality (MMR). Among them by increasing access to quality blood services which is carried out through collaboration between the Puskesmas, the Indonesian Red Cross (PMI) and the Hospital. This study aims to evaluate the Quick Wins program with elements of input, process and output with a qualitative design presented descriptively. Initial informants were determined by purposive sampling technique. The method of data collection was through in-depth interviews, observation and documentation of 4 initial informants consisting of coordinating midwives, health promotion workers, village midwives and cadres and 1 triangulation informant namely the head of the family health section of the Brebes district health office. The results of the study show that the input element has been maximized both from Human Resources (HR), funding or infrastructure. In the process element, the program has been implemented well, as evidenced by facilitating the adequacy of potential donors to replace the main donors whose families are pregnant women/Substitute Blood Donors (DDP). In terms of output, 50% of the Quick Wins program at the Paguyangan Health Center has made some village areas as foster villages, namely Taraban, Pagojengan, Ragatunjung, Kretek, Cilibur and Cipetung villages while the other 6 villages are still in the processKeywords : Quick Wins, donor recruitment, effort, AKI

Abstract

Premature Rupture of Membranes (PROM) is a condition where the amniotic membranes rupture before signs of labor appear. Cases of premature rupture of membranes at Brebes District Hospital are cases that are often encountered; this case reached 273. The impact of PROM has a high risk to the continued health of the mother and fetus. The aim of this research is to analyze midwifery care comprehensively using Varney's 7 steps. This research uses a qualitative descriptive method with a case study approach. Data collection used interview methods, observation, physical examinations, supporting examinations, medical records, KIA books, and other literature. The main informant was Mrs. “J”, while the additional informants were the coordinating midwife, general practitioner, obstetrician, laboratory officer, husband, and family. Based on the results, Mrs. “J” experienced problems during labor with premature rupture of membranes. Based on the results, Mrs. “J” at the time of childbirth had problems with the cranium being broken prematurely after anamnesies and continuous examination by midwife by conducting an in-depth examination with thin portio results, opening 10 cm, UUK point, cranial membrane (-), presentation of the head, and head-down hodge IV. The baby is born spontaneously, with strong crying and normal reflexes
